摘要
目的观察L-精氨酸对胎儿生长受限(FGR)脐动脉血流及胎盘Bcl-2、Bax凋亡蛋白表达的影响,探讨L-精氨酸对FGR的疗效。方法选择FGR患者60例,随机分为常规组30例和L-精氨酸组30例。孕期定期应用B型超声监测二组胎儿脐血流参数;分娩后称新生儿体质量,10min内取胎盘中央组织,100g/L甲醛固定后石蜡包埋,免疫组织化学法检测各组胎盘Bcl-2、Bax凋亡蛋白的表达。采用SPSS11.5软件进行统计学分析。结果L-精氨酸组新生儿平均出生体质量显著高于常规组(P<0.01)。治疗后,与常规组比较,L-精氨酸组快速血流比值较高(P<0.05),搏动指数、阻力指数较低(Pa<0.05)。常规组较L-精氨酸组Bax表达量相对增强,而Bcl-2表达相对减弱。结论L-精氨酸可降低胎盘细胞凋亡相关基因Bax蛋白的表达,增加Bcl-2的表达,减少胎盘细胞凋亡,改善胎盘功能,促进胎儿发育。
Objective To explore the effect and mechanism of L - arginine on fetal growth restriction by observing the expression of Bcl - 2 and Bax in placenta. Methods Sixty patients with FGR were chosen,among which 30 cases who were treated with conventional ways were as convention group, and the other 30 cases who were treated with L - arginine were as L - arginine group. The birth weight and perinatal fetus outcome were detected. The central tissue of placenta got within 10 min after delivery were fixed by 100 g/L formaldehyde and embed by paraffin wax to observe the expression of Bcl - 2 and Bax using immunohistochemistry. SPSS 11.5 software was used to analyze the data. Results Compared with convention group,the birth weight of L - arginine group was higher( P 〈 0. 01 ). Compared with convention group, the fast blood velocity rate of L - arginine group was higher( P 〈 0.05 ), while the pulse index, resistant index after treatment in L - arginine group were lower than those in convention group(Pa 〈0.05). The expression of Bax in convention group increased compared with that of L - argi- nine group,but that of Bcl - 2 decreased. Conclusions L - arginine can degrade the expression of Bax protein,while promote the expression of Bcl - 2 which could contribute to decrease apoptosis, improve the function of placenta, increase the development of fetal.
