摘要
目的:探讨多层螺旋CT bolus tracking技术对肝脏门静脉成像的价值。方法:100例健康人按触发点阈值随机分为5组,使用bolus tracking技术行全肝增强扫描,测量腹主动脉、门静脉、肝实质的强化CT值及各组到达设定阈值的时间,并进行门静脉三维重建(MIP及VR法)和成像质量评价。结果:门静脉与肝实质CT值差(PV-L值):第1组至第5组分别为(37.39±10.46)Hu、(65.98±18.75)Hu、(96.21±15.38)Hu、(75.14±14.57)Hu、(52.39±15.46)Hu;延迟时间第1组至第5组分别为(31.39±3.46)s、(35.79±4.17)s、(40.50±4.99)s、(45.25±5.09)s、(49.92±6.34)s;门静脉成像质量优秀率第1组至第5组分别为30%、65%、95%、80%、40%。结论:多层螺旋CT bolus tracking技术门静脉成像方法能监测个体差异,明显提高门静脉5级以上分支的显示率;第3组触发点阈值15 Hu为其最佳扫描阈值。
Objective:To evaluate the value of using Bolus tracking trigger scan technology in muhi-slice spiral CT portography (MSCTP). Methods :100 patients were divided into five groups randomly by different threshold value. The whole liver was scanned with bolus tracking trigger scan technology in portal-venous phase with enhanced. The average added value of contrast enhancement in abdominal aorta,liver parencbyma, portal vein, P-L value and the delay time of each group was measured. MSCT rendering for order branches of portal vein in every case, and the quality of the pictures were evaluated. Results: The maximum density differences between the portal vein and liver (PV-L) in five groups were (37.39±10.46) Hu,(65.98±18.75) Hu,(96.21 ±15.38) Hu,(75.14±14.57) Hu,(52.39± 15.46) Hu,and the time reaching were (31.39 ±3.46) s,(35.79 ±4.17) s,(40.50 ±4.99) s,(45.25 ±5.09) s, (49.92 ± 6.34) s. The visualization rates of up-fifth order branch of portal vein were 30 % ,65 % ,95 % ,80 % ,40 %. Conelusion:MSCTP with bolus tracking trigger scan technology can monitor individual differences and improved visualization rates of fifth-sixth order branch of portal vein, and threshold value in group 3 ( with 15 Hu added to the background ) is the best for MSCTP
