输尿管上段结石体外冲击波碎石治疗失败后的输尿管镜治疗 被引量:8

摘要 目的探讨输尿管上段结石体外碎石失败后的微创治疗手段。方法对57例首先ESWL治疗失败后的输尿管上段结石患者行输尿管镜下气压弹道碎石取石术。结果57例患者中,49例经过行输尿管镜一次粉碎或取出结石,6例部分较大结石碎屑冲入肾盂,2例结石完全冲入肾盂。其中5例术后再次给予ESWL治疗1、2次,2例术后行保守治疗,1例第2次ESWL后再次行输尿管镜术,出院2~4周后复查结石均完全清除。发生输尿管穿孔1例,输尿管黏膜撕裂1例。结石清除时间5~45d不等,平均11.7d。结论输尿管镜术是对ESWL治疗输尿管上段结石失败后的有效治疗手段,具有治疗周期短、并发症少的优点。 [Objective] To explore the best treatment of upper ureteral stones which treated by ESWL ineffectively. [Methods] Before the use of ureteroscopy, ESWL was employed in the treatment of 57 cases of upper ureteral stones. [Results] The operation succeeded firstly in 49 of the 57 cases, residual stones appeared in 8 cases. Then, ESWL was employed once or twice in 5 cases; conservative treatment was employed in 2 cases; ureteroscopy was used again in 1 case after the 2nd ESWL. All the stones were expelled 2-4 weeks after discharge. Ureter perforation occurred in 1 case, ureteral mucosa avulsion in 1. The time for complete stone expelling was 5 to 45 days, and the average time was 11.7 days. [ Conclusions] Ureteroscopy offers higher efficacy and lower invasion and is an effective measure for the treatment of upper ureteral stones after failure of ESWL.
