摘要
目的探讨人乳头瘤病毒(human papillomavirus,HPV)DNA的病毒载量检测在亚临床型女性宫颈癌筛查中的应用价值。方法拟设计4条分别针对HPVl6,18,33,58与人肛球蛋白的特异性引物与探针,建立一种新型的荧光定量PCR方法,对541例亚临床型女性宫颈组织细胞进行HPV型别和病毒栽量检测。结果541例样本中p球蛋白结果阳性的标本521例,其中56例HPVDNA阳性。HPV16,HPV18型以及多重感染的阳性检出率分别为46.42%(26/56),23.21%(13/56),30.36%(17/56);感染的最低病毒栽量分别为≥10。拷贝/“细胞”,≥10^4拷贝/“细胞”,≥10^2拷贝/“细胞”;单一型别HPV感染的病毒载量高于多重感染的病毒载量,且二者之间差异有统计学意义(P〈O.05)。结论HPVl6,HPVl8是亚临床女性感染HPV的主要型别,HPVDNA病毒载量的检测为宫颈癌的常规筛查提供了一个动态监测手段。
Objective To study the application value of HPV (human papillomavirus) DNA detection of viral load at cervi- cal cancer screening for women without clear clinical symptom. Methods To design HPV16, 18, 33,58 and Human globin specific primers and probes separately,and set up a new fluorescent quantitative PCR method to detect HPV types and viral load on cervix tissue sample from 541 patients. Results 521 out of 541 samples were 13-globin gene positive,and the HPV DNA of 56 out of the 521 samples were positive on HPV-DNA. The HPV DNA infection rates of HPV16 ,HPV18 and multiple positive infection were 46.42% (26/56),23.21/% (13/56), 30.36/% (17/56) separately;the lowest viral loads of HPV16,HPV18 and multiple positive infection were ≥10^3 copies/"cell",≥10^4 copies/"cell",≥10^2 copies/"cell" sepa- rately;viral load of single HPV infection was higher than the HPV multi-infection,and there was a significant difference (P〈0. 05) between them. Conclusion HPV16,HPV18 are the main types of HPV infection in females without clear clini- cal symptom,the viral load testing provide a dynamic monitoring indicators for routine cervical cancer screening.
