全膝关节置换围手术期应用骨关节角度测量器的价值 被引量:1

Application of orthopedics tool measurement apparatus during perioperative period of total knee replacement

摘要 背景:临床检查工具的标准化和使用简便是对现代临床测量诊断工具的基本要求。目的:以国际上公认的Cybex Norm测量系统为标准对比测量方法,对关节角度测量器进行四个维度的临床评估和检验,以验证其科学性、可重复使用性和实用性。设计、时间及地点:随机对照试验,于2004-10/2006-03在北京协和医院骨科完成。对象:选择全膝关节置换围置换期膝骨性关节炎患者47例(66膝),男11例,女36例:年龄56-82岁,平均64岁。随机分为OTMA组(n=24)33膝;Cybex组(n=23)33膝。方法:四个纬度的检验方法:与国际标准测量器Cybex Norm测量系统的测量结果进行比较,证明该测量器的准确性;选取不同年资的医生进行测量工作,明确该测量器的使用者范围:在治疗前、治疗后3个月和1年随访时,分别应用关节角度测量器和Cybex Norm型等速测量系统对两组患者进行关节活动度测量。将关节活动度测量结果在膝关节HSS量表中应用,并填写目测类比疼痛评估量表及SF-36生活质量评估量表,证实该测量器的适用范围和可重复使用性及实用价值,将测量结果与主观量表评估结果进行相关性分析,以证实关节角度测量器的评估效能。主要观察指标:两组不同时间点关节活动度测量结果《值》的符合率;各量表得分及关节活动度间的相关性。结果:评估完成率为100%;关节角度测量器的测量结果与Cybex Norm测量结果之间的符合率为98.3%;两组中HSS量表评分值随关节活动度增加而升高(P〈0.01),相关分析结果显示关节活动度与量表结果高度相关,符合患者的临床表现。结论:关节角度测量器测量结果准确,能反映患者的临床症状,测量过程能被患者所接受,并可进行重复测量使用,能够实现测量检查辅助临床诊断之目的。 BACKGROUND: Standardization and convenience are basic requirements for the tools of clinical measurement and diagnosis. OBJECTIVE: To verify its scientific, reusable and practical as the standard measurement of orthopedics tool measurement apparatus (OTMA) according to the results of Cybex Norm measurement system for the four dimensions of clinical evaluation and testing. DESIGN, TIME AND SETTING: A randomized controlled study was performed at the Department of Orthopedics, Peking Union Medical College Hospital from October 2004 to March 2006. PARTICIPANTS: A total of 47 cases (66 knees) with osteoarthritis of knee, undergoing total knee replacement (TKR), comprising 11 males, 36 females, at the age of 56 to 82 years old, with an average of 64 years. They were randomly divided into an OTMA group (n=24, 33 knees) and a Cybex group (n=23, 33 knees). METHODS: Four dimensions of clinical evaluation and test: compare with the measurement results from the international standard gauge of the Cybex Norm measurement system, the accuracy of the measuring device was proved. Various doctors for years of survey work were selected, and the measuring range of users was clear. Before treatment, after 3 months and 1-year follow-up, OTMA and the Cybex Norm Isokinetic measurement-based system was used to measure range of motion in two groups. Measurement would be the results of joint activities in the Hospital for Special Surgery Knee Score (HSS) scale, and visual analog pain scale and SF-36 quality of life scale were also applied. These confirmed that the scope of application of measuring and re-use and practical value. Correlation of measurement results with the subjective scale was analyzed to assess the performance of OTMA device. MAIN OUTCOME MEASURES: Coincidence rate of measurement results of joint activities "value" was measured at different time points in both groups. Correlation between scale scores and range of motion was determined. RESULTS: The completion rate of the assessment was 100%. The coincidence rate of joint angle measurement device and the Cybex Norm measurement results was 98.3%. In both groups, the HSS scale score increased with the increased range of motion (P 〈 0.01 ). Correlation analysis showed that range of motion are highly relevant to scale results, which was accorded with the clinical manifestations in patients. CONCLUSION: Measurement of OTMA results are accurate, reflect the patient's clinical symptoms, can be accepted by patients, and the measurement can be repeated and supper the clinical diagnosis.
