摘要
目的了解农村慢性心力衰竭患者用药依从性,探讨影响其用药依从性的因素,以提出针对性的护理对策。方法采用一般资料问卷、用药相关知识及态度问卷、服药依从性及自我管理问卷和家庭功能问卷,对100例农村慢性心力衰竭患者进行调查,并采用多因素回归分析患者用药依从性的影响因素。结果本组患者用药依从性差,表现为不按要求的次数、量、时间服药和不能长期坚持服药;服药依从性受患者年龄、心功能分级、家庭功能类型、病程、对疾病及药物相关知识的掌握程度、对疾病的态度影响(P<0.05)。结论农村慢性心力衰竭患者普遍服药依从性差。影响患者用药依从性的因素较多,心功能愈差、病程越长,患者用药依从性越差;患者对疾病和药物知识了解、良好的家庭功能、有效的自我管理行为等均可显著提高患者的用药依从性。
Objective To understand the compliance to medication among chronic heart failure patients in rural area, explore the influencing factors, and thus to formulate directed effective nursing strategies. Methods By means of questionnaires concerning the general information, knowledge related to and attitude toward medication, compliance to medicine taking and self-management, and family functioning, a survey was made with 100 rural-area patients with chronic heart failure, and then a multiple regressive analysis was made of the factors influencing their compliance to medicine taking. Results The medicine taking compliance among the subjects in this study was poor, which took the forms of failing to follow the specified times, dosage, frequency and/or duration of medicine taking. Their compliance in this respect was influenced by such factors as the patient＇s age, cardiac function, family functioning type, duration of the disease, knowledge related to the disease and medication, and attitude toward the disease （P〈0.05）. Conclusion The compliance to medication among chronic heart failure patients in the rural area is poor. And the factors influencing their compliance were various. The poorer their cardiac function and the greater the duration of the disease, the poorer their compliance. The patient＇s good knowledge related to the disease and medicine taking, sound family functioning, effective self-management, can markedly increase their compliance.
