摘要 目的:探讨心脏手术期间在心肺转流(CPB)及非CPB情况下患者膀胱温度监测的可行性及其临床意义。方法:40例行择期心脏手术患者分为CPB组(A组)和非CPB组(B组),每组20例。术中分别监测血液、膀胱和直肠的温度,A组各记录时点分别为Swan-Ganz导管置入(T0)、手术切皮(T1)、CPB开始(T2)、CPB后10min(T3)、CPB后30min(T4)、复温开始(T5)、复温后10min(T6)、复温后30min(T7)、CPB停止(T8)、手术结束(T9);B组为Swan-Ganz导管置入(T0)、手术切皮(T1)、开胸(T2)、开胸后10min(T3)、开胸后30min(T4)、开胸后60min(T5)、开胸后90min(T6)、开胸后120min(T7)、关胸(T8)、手术结束(T9)。结果:A组膀胱温度与血液、直肠温度变化均呈显著正相关(r=0.89,P<0.01),B组膀胱温度与血液、直肠温度变化也均呈显著正相关(r>0.84,P<0.01)。结论:无论是在CPB还是在非CPB情况下,膀胱温度都与血液、直肠温度有显著的正相关性,能反映心脏手术期间患者核心体温变化情况。 Objective To study the feasibility of monitoring urinary bladder temperature during heart surgery under cardiopulmonary bypass(CPB) or not. Methods Forty patients scheduled for heart surgery were devided into two groups,CPB group (Group A,n = 20) and not CPB (Group B,n = 20).Temperature of blood, uninary bldder and rectum were recorded after Swan-Ganz catheter insertion(T0), afer skin incision(T1),after CPB(T2), 10 min after CPB (T3) ,30 min after CPB(T4),after rewarming(T5), 10 min after rewarming(T6), 30 rain after rewarming(T7),and at the end of CPB (T8) and operation ( T9 ) in Group A, and after Swan-Ganz catheter insertion (T0), afer skin incision ( T1 ), after sternum splitting (T2), 10 min after sternum splitting (T3), 30 rain after sternum splitting (T4), 60 min after sternum splitting (T5), 90 min after sternum splitting (T6), 120 min after sternum splitting (T7), and at the end of sternum sewing (T8)and operation (T9)in Group B. Results The correlation between urinary bladder temperature and blood temperature, and rectum temperature were significant(r 〉 0.89,P 〈0.01)in Group A. The correlation between urinary bladder temperature and blood temperature, and rectum temperature were siguificant(r 〉 0.84, P 〈0.01 )in Group B. Conclusion The positive correlation between urinary bladder temperature and blood temperature, and rectum temperature, and oesophagus temperature were significant whether under CPB or not. Monitoring urinary bladder temperature could reflect the change of core body temperature during heart surgery.
