摘要
目的探讨针刺内关穴对缺血再灌注损伤心肌的保护作用及与血浆β-内啡肽的相关性。方法将大鼠针刺5 d后麻醉捆绑,记录心电图。空白组同期捆绑对照;假手术组开胸挂线不结扎;模型组、轻手法针刺组和重手法针刺组开胸结扎大鼠左冠状动脉前降支,造成心肌缺血20 min,再灌注40 min。造模后颈动脉取血,测定血清缺血修饰白蛋白(IMA)和血浆β-内啡肽的含量。结果模型组大鼠心肌缺血再灌注后心电图ST段电位值明显高于空白组、假手术组,差异具有统计学意义(P<0.01),也高于轻手法针刺组、重手法针刺组,差异均具有统计学意义(P<0.01);模型组IMA、β-EP含量较空白组和假手术组明显升高,差异具有统计学意义(P<0.01),而轻手法针刺组和重手法针刺组与之比较显著降低,差异具有统计学意义(P<0.01);重手法针刺组大鼠心肌缺血再灌注后的心电图ST段电位值、IMA和β-EP含量均低于轻手法针刺组,差异具有统计学意义(P<0.05)。结论针刺内关穴能抑制心肌缺血再灌注大鼠血浆β-EP的升高,减轻应激反应的发生,从而起到保护心肌的作用。而重手法针刺组的针刺效果优于轻手法针刺组。
Objective To investigate the mechanism of acupuncturing point Neiguan to protect the myocardial ischemia and reperfusion injury and the effect of β-EP. Methods After five days of acupuncture, all rats were bound in anesthesia and their ECG were recorded in whole process. The rats in blank group were bound only, in sham operation group opened their thoracic cavity with no ligation and in model, mild manipulation and strong manipulation groups opened their thoracic cavity to ligature left descending coronary artery which caused myocardial ischemia in 20 minutes and then reperfusion in 40 minutes. Then, carotid blood was taken to be measured the volumes of IMA and β-EP. Results The ST Ⅱ of ECG in model group was higher than that in blank group and sham operation group obviously (P〈0.01), and lowerthan that in mild manipulation and strong manipulation groups (P〈0.01). The IMA and β- EP content in model group were increased significantly compared with the blank group and sham operation group (P 〈0.01). The ST Ⅱ of ECG, IMA and β -EP in strong manipulation group were lower than that in mild manipulation group (P〈0.05). Conclusion Acupuncturing point Neiguan can decrease the β-EP content of myocardial ischemia and reperfusion rats to protect the heart muscles.
