摘要
目的探讨系统性红斑狼疮(SLE)患者外周血催乳素(PRL)水平、Th1/Th2细胞因子分泌模式与SLE患者病情活动的关系。方法检测40例SLE患者(SLE组)血清PRLIFN-y、IL-4水平,分析二者与SLE病情活动的关系并与20例健康献血者(对照组)比较。结果①SLE组较正常对照组血清PRL[(21.58±4.29).g/ml与(11.87±2.57)ng/ml,P〈0.01)]、IL-4[(26.79±5.08)ng/L与(10.71±1.35)ng/L,P〈0.01)]高,而IFN-γ[(11.47±3.36)ng/L与(18.36±2.61)ng/L,P〈0.01)]、IFN-γ/IL-4(0.76±0.29与2.30±0.15,P〈0.01)较正常对照组低。②SLE病情活动组PRL[(28.07±6.36)ng/ml与(14.61±2.14)ng/ml,P〈0.01)]、IL-4[(38.52±8.44)ng/L与(14.15±1.63)ng/L,P〈0.01)]高于病情稳定组,而IFN-γ[(6.98±2.72)ng/L与(16.24±2.57)ng/L,P〈0.01)]、IFN-γ/IL-4(0.35±0.14与1.24±0.29,P〈0.01)水平低于病情稳定组。③SLE活动期组10例患者治疗好转后较治疗前PRL[(39.41±11.65)ng/ml与(14.49±8.65)ng/ml,P〈0.01)]、IL-4[(45.12±10.44)ng/L与(17.53±5.42)ng/L,P〈0.01)]下降,IFN-γ[(6.31±2.59)ng/L与(16.89±4.43)ns/L,P〈0.01)]、IFN-γ/IL-4(0.16±0.11与1.16±0.27,P〈0.01)回升。结论SLE患者存在高PRL血症及Th2细胞因子优势,高PRL血症和Th1/Th2比值失衡程度与病情活动呈消长关系。
Objective To investigate the correlation of the serum prolactin level and the secret mode of Thl/Th2 cytokines with clinical activity in patients with systemic lupus erythematosus. Methods The serum level of PRL, IFN-γ and IL-4 in 40 patients were measured and the relationship between them were analyzed . Results (1) The levels of PRL[ (21.58 ± 4.29 ) ng/ml vs ( 11.87 ± 2.57 ) ng/ml, P 〈 0.01 ), IL-4 [ ( 26.79 ± 5.08 ) ng/L vs (10.71 ± 1.35 )ng/L,P 〈 0.01 ] in SLE patients were significantly higher than the healthy controls, but IFN-γ [ ( 11.47 ± 3.36) ng/L vs ( 18.36 ± 2.61 ) ng/L, P 〈 0.01 ], IFN-γ/IL-4 (0.76 ± 0.29 vs 2.30 ± 0.15, P 〈 0.01 ) were lower than controls. (2)The levels of PRL[ (28.07 ± 6.36)ng/ml vs ( 14.61 ± 2.14) ng,/ml,P 〈0.01 ) ] ,IL-4 [ ( 38.52 ± 8.44 ) ng/L vs ( 14.15 ± 1.63 ) ng/L, P 〈 0.01 ] in the active SLE patients were significantly higher than that in the nonactive patients,whereas,the levels of IFN-γ[ (6.98 ±2.72) ng,/L vs ( 16.24 ±2.57)ng/L,P 〈0.01 ] and IFN-γ/IL-4 (0.35 ± 0.14 vs 1.24 ± 0.29 ,P 〈 0.01 ) were lower in the active group compared with the nonactive patients. (3)The levels of PRL [ ( 39.41 ± 11. 65 ) ng/ml vs ( 14.49 ± 8.65 ) ng/ml, P 〈 0. 01 ) ] and IL-4 [ (45.12 ± 10.44 ) ng/L vs ( 17.53 ± 5.42 ) ng/L, P 〈 0.01 ] declined while IFN-γ[ ( 6.31 ± 2.59 ) ng/L vs ( 16.89 ± 4.43 ) ng/L,P 〈 0.01 ] and IFN-γ/IL4 ( 0.16 ± 0.11 vs 1.16 ± 0.27, P 〈 0.01 ) increased when SLE patients in remission. Conclusions Hyperprolactinemia and imbalance of Th cytokines production which exhibited Th2 dominant are found in SLE patients. Prolactin and the degree of imbalance of Th cytokines production varies with the remission or exacerbation of the disease.
