胰岛素联合5-氟尿嘧啶对H22肝癌移植瘤裸鼠的化疗增效作用

摘要 目的:观察胰岛素能否增强5-氟尿嘧啶(5-FU)对裸鼠的化疗疗效,并探讨血清胰岛素样生长因子1(IGF-1)水平在其增效机制中的意义。方法:60只裸鼠建立H22肝癌移植瘤模型后,随机分为6组,每组10只:对照组、单用大剂量胰岛素组、5-FU组、大、中、小剂量胰岛素(4.5、3.0、1.5U/kg)联合5-FU组。5-FU按10mg/kg腹腔注射,每天用药1次,连用8d。观察各组抑瘤率,并用ELISA方法检测裸鼠血清中IGF-1和胰岛素样生长因子结合蛋白3(IGFBP-3)含量。结果:4组肿瘤生长抑制率比较,差异有统计学意义(F=71.410,P<0.001),且大剂量胰岛素联合5-FU组对H22肝癌移植肿瘤生长抑制率达(51.35±3.14)%,高于5-FU组的(27.93±2.35)%(P<0.05)。4组血清IGF-1水平比较,差异有统计学意义(F=13.720,P<0.001),且大剂量胰岛素联合5-FU组血清IGF-1水平[(492.13±58.88)μg/L]低于5-FU组[(595.51±260.77)μg/L](P<0.05)。结论:在裸鼠H22肝癌移植瘤模型中,胰岛素能增强5-FU化疗疗效,其增效机制可能与IGF-1水平有关。 Aim: To observe whether insulin can enhance chemotherapeutic response of 5-fluorouracil (5-FU) and study the significance of serum insulin-like growth factor-1 (IGF-1) level in the enhancement. Methods:A total of 60 nude mice transplanted H22 liver eancer were built and randomly divided into 6 groups: control group, high dose insulin alone group, 5-FU alone group, and 5-FU + high, medium, low dose insulin (4.5,3.0,1.5 U/kg) groups. 5-FU was given at 10 mg/kg body mass intraperitoneally. Each mouse was injected a corresponding dosage once a day for 8 d. The rate of tumor inhibition was calculated. Serum IGF-1 and insulin-like growth factor binding protein-3 (IGFBP-3) were determined by using ELISA methord. Results :There was significant difference on the inhibition rate of tumor growth among four groups (F =71.410, P 〈 0. 001 ). The inhibition rate of tumor growth in high dosage of insulin + 5-FU group was (51.35 ± 3.14) %, which was significantly higher than ( 27.93 ± 2.35 ) % in 5-FU alone group ( P 〈 0.05 ). There was significant difference on serum IGF-1 level among four groups ( F = 13. 720, P 〈 0. 001 ). Serum IGF-1 level in high dose insulin + 5-FU group [(492.13±58.88) μg/L] was significantly lower than that in 5-FU alone group [(595.51 ±260.77) Ixg/L] (P 〈0.05). Conclusion: Insulin could enhance chemotherapeutic response of 5-FU in nude mice with transplanted H22 liver eaneer. The decreasing level of serum IGF-1 may play an important role in the enhancement of insulin on 5-FU.
