摘要
目的探讨检测2型糖尿病(T2DM)合并高血压(HT)患者外周血中抵抗素的意义。方法选择2006年3月至2007年12月内分泌科与心脏科住院患者为研究对象,并设对照组。入选患者分为4组:单纯T2DM(A组,28例),T2DM合并HT组(B组,26例),单纯HT组(C组,30例),健康对照组(对照组,25例)。分别检测4组的空腹血糖(FBG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)以及抵抗素水平,同时应用颈动脉超声测量颈内/外动脉的最大内中膜厚度(IMT),并探讨它们之间的关系。结果HT合并T2DM组患者外周血抵抗素水平明显高于其他3组〔(39.0±16.4)ng/ml vs(14.5±8.8)ng/ml、(14.3±5.6)ng/ml、(10.3±3.3)ng/ml,P均<0.05〕,对该组进行以血糖为因变量的多元回归分析发现,血糖的变化与年龄、BMI、舒张压和收缩压及血清抵抗素水平有关,进行年龄、BMI和血压调整后,血糖水平仍然与外周血抵抗素水平有关。结论外周血抵抗素水平是HT合并T2DM的独立危险因子。
Objective To investigate the value of detecting resistin level in peripheral blood of patients with coexistence of type 2 diabetes (T2DM) and hypertension (HT). Methods Eligible patients were all admired in departments of endocrinology and cardiology from March 2006 to December 2007. Following groups were studied:T2DM alone group (group A, n = 28), T2DM complicated with HT group (group B, n=26), HT alone group (group C, n =30), normal control group (control group, n =25). Fasting blood glucose (FBG), HbAlc, cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), and serum resistin were detected in separate groups; the maximum intra-mid membrane thickness (IMT) of inner/outside carotid artery was measured as well; the relations among above indexes were studied. Results The resistin level in group B was obviously higher.than that in other three groups[ (39. 0 ± 16. 4) ng/ml vs ( 14. 5 ±8. 8 ) ng/nd, ( 14. 3 ± 5.6) ng/ml, ( 10. 3 ± 3. 3 ) ng/ml, respectively, all P 〈 0. 05 ). The glucose level showed correlation with age, body mass index (BMI) ,systolic/diastolic blood pressure and serum resistin level by multiple regression analysis; the glucose level was still correlated with serum resistin level after adjusting age, BMI and blood pressure. Conclusions Resistin level in peripheral blood is an independent risk factor of coexistence of T2DM and HT.
