摘要
目的探讨小儿肾功能衰竭(RF)时血液透析(HD)技术的临床应用效果及HD前后患儿生化指标的变化。方法采用COBE CentrsystemⅢ型透析机,空心纤维透析器,直接穿刺动、静脉来建立血管通路,碳酸氢盐透析液;透析时间1.5~4h,间隔时间1~12天;透析次数1~9次。全身肝素化抗凝。对29例急性肾功能衰竭(ARF)患儿和10例慢性肾功能衰竭(CRF)患儿共进行92次HD治疗,并分别于透析前、透析结束前15分钟及出院时抽取静脉血检测血清BUN、CRE、电解质,同时检测动脉血气分析。结果RF患儿透析后血清尿素氮(BUN)肌酐(CRE)显著下降(P<0.001),同时酸中毒和电解质紊乱得到纠正或改善。ARF患儿经过1~5次透析后24例临床治愈,血BUN、CRE恢复正常,5例明显好转;CRF经1~9次透析好转,但不能长时间维持,须维持性HD。结论HD是RF患儿有效的急救治疗措施,HD能减少透析患儿发生严重并发症。
Objective To compare the biochemical parameters before and after hemodialysis, and to asses the effects of hemodialysis (HD) treating renal failure (RF). Methods Using dialyzer type COBE centrsystem, the treatment was performed with soda solution through arterial and venous reacts. Twenty-nine children with acute renal failure (ARF) and ten children with chromic renal failure (CRF) were given hemodailysis treatment.the total quantity of which was ninety-two.The took systemic heparin for anticoagulation.Serum BUN, CRE and electrolytes were measured at fifteen minutes per-and postdialysis,and arterial blood gas was analyzed. Results Serum BUN and CRE of the children with RF decreased significantly after hemodailysis,and acidosis and electrolyte's disorder were corrected at the same time,Among those ARF patients, twenty-four of them achieved clinical recovery with normal level of serum BUN and CRE,after undergoing once to five times hemodialysis .Five of them were discharged with deter conditions.However, for the CRF children reparative hemodialysis (once to nine times) could improve their conditions, and consistent hemodialysis was needed to maintain the effect .Results;HD is a kind of effective performance rescuing children with RF, which could decrease the decrease the severe complications about dialysis.
