超声造影定量评价犬慢性缺血性肾病肾皮质血流灌注改变 被引量:1

Quantitative evaluation of contrast-enhanced ultrasound in the diagnosis of chronic ischemic renal disease of dogs

摘要 目的探讨超声造影定量分析技术在评价犬慢性缺血性肾病模型肾皮质血流灌注改变中的应用价值。方法健康成年犬5只,用Ameroid环制作犬右肾动脉慢性狭窄模型。每只犬采用建模前后自身对照观察,建模后每隔1周对犬右肾进行超声检查。采用QLAB定量软件测量犬肾皮质的超声造影灌注参数值,包括曲线下面积(area under curve,AUC)、达峰时间(time to peak,TTP)、峰值强度(derived peak intensity,DPI)、曲线上升支斜率(slope of ascending curve,A)、曲线下降支斜率(slope of descending curve,a)。每次超声检查前抽血测量血肌酐、尿素氮。结果随着犬慢性肾功能损害的发展,其时间-强度曲线上升缓慢,下降缓慢,到达峰值时间延迟。超声造影灌注参数表现为AUC、TTP、DPI、A逐渐增大,而a减小。其中DPI和TTP的差异出现统计学意义较早(建模后第4周),而A值最晚(建模后第7周)。血肌酐、尿素氮值的改变在建模后第11周出现统计学意义(P〈0.05)。结论超声造影定量分析技术能准确显示犬。肾慢性缺血性病变的血流灌注变化,较常规血肌酐、尿素氮灵敏。DPI和TTP是反映肾慢性缺血灌注改变的敏感指标。 Objective To evaluate the diagnostic value of contrast-enhanced ultrasound(CEUS) in chronic ischemic renal disease(CIRD) of dogs. Methods The dog's model of CIRD was established by placing the Ameroid ring on the distal portion of right renal artery through operation. CEUS were performed every one week after the operation. The slope rate of ascending curve(A), the slope rate of descending curve (a) ,area under curve (AUC),derived peak intensity(DPI),and time to peak(TTP) were measured in renal cortex of dogs using QLAB software package and statistically analyzed. Blood serum urea nitrogen(BUN) and serum creatinine(SCr) of dogs were also tested. Results With the progress of CIRD, dogs showed decreased enhancement and delayed perfusion in the renal perfusion curve. AUC, TTP, DPI and A were gradually increased,however, a was decreased. Earliest significant changes happened 4 weeks after operation on DPI and TTP, the latest significant change happened 7 weeks after operation on A and 11 weeks on BUN and SCr( P 〈0.05). Conclusions CEUS can accurately display the perfusion changes of CIRD model of dogs,it's more sensitive than the BUN and SCr. DPI and TTP are the most sensitive quantitative indexes of CEUS.
