摘要
目的:本研究采用改良淋巴结显示液(MLNRS)与免疫组织化学染色(IHC)联合法对胃癌术后标本进行区域淋巴结检查,分析其对病理分期(pTNM)的影响方法:以46例胃癌术后标本为研究对象。先行传统法检出淋巴结,再经MLNRS染色进一步检出淋巴结,所有淋巴结经HE染色诊断是否有转移,无转移者再经IHC法检测微转移。结果:传统法检出淋巴结共1 456枚,HE:染色130枚有转移,无转移者再经IHC法检测出63枚存在微转移标本再经MLNRS染色后又检出1 377枚淋巴结,其中HE染色106枚有转移,无转移者再经IHC法102枚存在微转移联合检查法淋巴结平均检出数及淋巴结转移检出率均较传统检查法明显增加,差异有统计学意义不同检查方法分组比较,pN分期以及pTNM分期结果差异明显,具有统计学意义结论:MLNRS能够提高胃癌区域淋巴结平均检出数,IHC法有助于检测淋巴结微转移,两者联合应用能够提高胃癌区域淋巴结转移检出率。
Objective： Modified lymph node revealing solution （MLNRS） was applied in combination with immunohistochemistry to examine the local lymph nodes in gastric cancer and the results were then used to reevaluate pathological staging according to the AJCC （sixth edition） N-staging criterion for gastric cancer. Methods： A total of 46 specimens obtained through standardized D2 gastrectomy were chosen for a prospec- tive study. AI! specimens were initially subjected to physical inspection of lymph nodes and then MLNRS stain- ing was used to further examine lymph nodes. All lymph nodes were examined by H＆E staining for evidence of metastasis, and negative nodes were subjected to IHC to detect micro-metastases. Results： The conven- tional preparation of lymph nodes yielded a total of 1456 lymph nodes, 130 of which were positive for metasta- sis according to evaluation of H＆E staining and an additional 63 of which were positive for metastasis by IHC. Further application of MLNRS identified 1377 additional lymph nodes, 106 of which were positive for metasta- sis according to evaluation of H＆E staining and an additional 102 of which were positive for metastasis by IHC. There was a statistically significant difference between the two methods in the detection of total number of positive lymph nodes. Conclusion： MLNRS can increase the detection rate of positive lymph nodes in gas- tdc carcinoma, and IHC is valuable for detection of lymph node mJcrometastases. The combination of MLNRS and IHC increases the detection rate of positive lymph nodes in gastric carcinoma, improving the accuracy of pTNM staging of gastric cancer.
