新近发生扩张型心肌病患者外周血淋巴细胞CD4^+/D8^+比值的测定及其与左室射血分数的相关性 被引量:3

摘要 目的探讨新近发生的扩张型心肌病患者外周血淋巴细胞CD4^+/D8^+比值同左室射血分数的相关性。方法对25例新近发生的扩张型心肌病患者(心肌病组)和22例体检健康者(对照组)通过流式细胞仪测定外周血淋巴细胞CD4^+和CD8^+的值,测算出CD4^+/D8^+比值,同时采用心脏超声测定扩张型心肌病患者的左室射血分数,对两组对象的上述参数作对比分析,并对心肌病组患者CD4^+/D8^+比值与左室射血分数行直线相关分析。结果心肌病组患者淋巴细胞CD8^+值明显高于对照组(22.35±2.43 vs 20.49±2.18,P〈0.05),CD4^+/D8^+比值显著低于对照组(1.90±0.27 vs 2.16±0.28,P〈0.01),但两组间CD4^+值并无明显差异(41.96±3.57 vs 43.81±3.59,P〉0.05)。心肌病组患者CD4^+/D8^+比值与左室射血分数呈显著负相关(r=-0.65,P〈0.01)。结论新近发生的扩张型心肌病患者,其CD4^+ T淋巴细胞水平与健康成人相似,但CD8^+ T淋巴细胞水平则明显下降。其心功能下降与免疫功能紊乱相关。 Objective To investigate the correlation of the T-cell CD4^+/D8^+ ratio with left ventricular ejection fraction in patients with recent-onset dilated cardiomyopathy (DCM). Methods Twenty-five patients with recent-onset dilated cardiomyopathy (≤6 months of symptoms) and 22 healthy individuals were enrolled in the study. Peripheral blood T-lymphocyte CD4^+/D8^+ ratio was measured by flow cytometry, and echocardiography was performed for assessment of left ventricular ejection fraction (LVEF). The relationship between CD4^+/D8^+ ratio and the left ventricular ejection fraction was analyzed by simple linear correlation. Results There was no significant difference in CD4^+ count between DCM and control groups (t=1.7675, P〉0.05); while the CD8^+ in DCM group were lower than those in control group (t=2.7662, P〈0.05). The CD4^+/D8^+ ratio in DCM group was higher than that in control group (t=3.2298, P〈0.01). The CD4^+/D8^+ ratio was negatively correlated with left ventricular ejection fraction in DCM group (Y=0.6014-0.1366X, r=-0.65, t=4.1355, P〈0.01). Conclusion Peripheral blood T-cell CD4^+/D8^+ is decreased in patients with dilated cardiomyopathy, and CD4^+/D8^+ ratio is correlated with left ventricular ejection fraction.
