甲烷和氦测定有效肺泡容量的对比研究

摘要 在肺弥散功能测试中,探讨从甲烷(CH_4)替换氦(He)作为示踪气体对有效肺泡容量 V_A 测定的影响。采用一次呼吸法,分别通过氦分析仪及快速红外分析仪,对30例不同肺部疾病患者进行 He 稀释法和 CH_4稀释法自身对比观察。结果表明,有效肺泡容量结果分别为5.04±1.35(V_ACH_4)和4.88±1.24L(V_AHe),V_ACH_4值略大,但相关系数仍达0.7679。其 CO 弥散量和弥散常数也相关良好。结论认为,He 和 CH_4虽溶解度及分子量有差别,但在肺泡容量及弥散量测定上无影响,也不受气流阻塞及气体分布不均的影响。 To observe whether methane (CH4) substituting for traditiional inert tracer gas helium (He) would influence the measurement of effective alveoar volume (V_A) during pulmonary diffusion capacity test or not single breath He dilution and CH4 dilution methods were conducted to obtain relevent VA and compared in 30 patients with pulmonary disease.The effective alveolar volume obtained by CH_4 dilution was 5.04_+1.36L,a little bit higher than 4.88±1.24L by He dilution,but stil correlated well,as r value equaled to 0.7679,diffusion capacity (DLCO)and diffusion constant (D_LCO/V_A) of two measurements were also correlated guite well.The results showed no significanl difference between the measurements using these two inert tracer gases,and not effected by airway obstruction and gas maldistribution.
