摘要
目的探讨云南白药气雾剂喷洒联合湿热敷治疗血液透析患者动静脉内瘘炎症的效果。方法选择2007年3月—2008年7月住院行血液透析发生动静脉内瘘炎症的患者120例,采用随机数字表法分为对照组和观察组各60例。从发生内瘘炎症开始,指导患者在透析24h后,用温水将内瘘处洗净。对照组用33%硫酸镁溶液湿敷4次/d,每次30min,如此反复至下次透析;观察组用40～50℃的温水湿热敷10～15min后,用云南白药气雾剂喷洒后按摩5～10min,再用保鲜膜包裹,保留3h,每日4次,如此反复至下次透析。结果两组动静脉内瘘炎症患者治疗4周后疗效比较,经秩和检验,差异具有统计学意义(u=7.048,P<0.05),观察组疗效明显优于对照组。结论采用云南白药气雾剂喷洒联合湿热敷治疗动静脉内瘘炎症,疗效较好,值得临床推广。
Objective To explore the effect of Yun＇nan White Medicinal Powder Spray together with hot-wet compression on arteriovenous fistula inflammation among hemodialysis patients. Methods 120 patients with hemodialysis-related arteriovenous fistula inflammation hospitalized between March 2007 and July 2008 were chosen as the subjects of this study and were, through random number tabling, divided into the observation group and the control group, each with 60. The patients were, from the very time when the inflammation occurred, guided to wash the fistula with warm water 24 h after the hemodialysis. The control group were asked to do compressions with 33% magnesium sulfate solution 4 times per day, each for 30 minutes, until the next hemodialysis. The observation group were instructed to apply Yun＇nan White Medicinal Powder Spray and massage for 5-10 minutes after doing the hot-wet compression with 40-50℃ water for 10-15 minutes and then wrap it with plastic film with the wrapping lasting for 3 h, 4 times per day, until the next hemodialysis. Results The difference of effect between the two groups after four weeks of treatment was statistically significant （u=7.048, P〈0.05）, with the effect for the observation group markedly superior to that of the control group. Conclusion Yun ＇nan White Medicinal Powder Spray together with hot-wet compression yields good effect on arteriovenous fistula inflammation among hemodialysis patients and is therefore worth clinical popularization.
