摘要
目的分析甲状腺微小良恶性病变的超声表现,评价超声在其诊断中的价值。方法对190例(212个结节)最大直径≤1.0cm的甲状腺结节进行研究,所有患者行二维及彩色多普勒超声扫描并经手术病理证实。结果甲状腺微小良恶性病变在形态、边界、声晕、回声强度、回声均匀度、囊性变,微小钙化、血流信号及伴发颈部淋巴结肿大均有统计学差异(P<0.05)。超声检查对于鉴别甲状腺微小良、恶性病变的敏感性、特异性、阳性预测值、阴性预测值分别为96.9%、90.5%、81.6%、98.5%,诊断准确率为92.5%。结论超声检查在甲状腺微小病变诊断和鉴别诊断中有重要的价值。
Objective To study the ultrasonographic features of micro-nodules of the thyroid and to evaluate the role of ultrasound in diagnosis of micro-nodules of the thyroid. Methods The clinical date were analyzed for 190 cases(212 nodules) of thyroid lesion which measured lcm or less in maximal diameter. All patients underwent surgery and had histopathological diagnosis. Results There was significant difference between benign and malignant micro-nodules of thyroid in the shape, margin, halo sign, echoic texture, cystic degeneration, microcalcifications, color Doppler flow signal and enlarged lymph nodes (P〈0.05).The sensitivity, specificity ,postive predictive value(PPV), negative predictive value (NPV) and overall accuracy is 96.9%, 90.5%, 81.6%, 98.5% and 92.5%, respectively. Conclusion Ultrasonography shows important value in the diagnosis of micro-nodules of the thyroid.
