摘要
为阻止角膜新生血管，改善角膜移植片的预后，用同种异体角膜缘上皮移植联合角膜移植术，对陈旧性眼化学烧伤５３例（５３眼），作干燥全角膜板层移植组（２１眼），和穿透性角膜移植组（１０眼）联合角膜缘上皮移植。以及对照组——单纯作全角膜板层移植组（２２眼）进行观察分析。结果：联合干燥全角膜板层移植者中，角膜上皮愈合时间为４～１４天，平均７．４天。４眼发生排斥反应。１５眼角膜上皮一直较稳定。植片保持透明者１５眼７１．４％。穿透性角膜移植组上皮愈合时间为３～５天，平均４天，９眼角膜上皮保持稳定。两组均有７０％的病例较有效地阻止角膜新生血管与假性胬肉的发生。对照组愈合时间为７～２０天，平均１３．７天。２２例中９只眼（４０．９％）发生上皮早期剥脱或持续上皮缺损。结论：对严重的眼部化学伤施行角膜缘上皮移植联合角膜移植是较有效的治疗方法。
Objective:To improve the prognosis of grafts by limbal transplantation for old ocular chemical burns.Methods:53 eyes with old chemical burns were divided into three groups.The combined procedure of limbal transplantation(LT) and lamelar keratoplasty with drypreserved corneal buttons(DLKP) was performed in 21 eyes,LT with penetrating keratoplasty(PK) in 10 eyes and routine lamelar keratoplasty(LK) in 22 eyes.Effects of three kinds of operation were compared.Results:In LT+DLKP group(21 eyes),the corneal epithelial wounds healed during 4 to 14 days(average 74 days),allograft rejection were identified in 4 eyes,the graft maintained stable and transparent in 15 eyes (714%).In LT+PK group(10 eyes),corneal epithelial wounds healed during 3 to 5 days(average 4 days),the graft maintained stable in 9 eyes.Whereas,in LK group(22 eyes),corneal epithelial wounds healed during 7 to 20 days(average 137 days),epithelial loss in early stage or persistent corneal epithelial defect were found in 9 eyes(409%).Conclusions:Combined procedure of LT and keratoplasty in an effective treatment for severe ocular chemical burns.
