摘要 目的探讨不同的吸氧方式获得同样效果的氧流量,寻找吸氧量效关系,为临床护理提供理论依据。方法对正常人群及严重烧伤患者进行分组观察,采用鼻管吸氧、鼻塞吸氧、面罩吸氧等不同的吸氧方式,以不同时间点(休克期、术前、术后)、不断改变供氧流量的方法,监测各种氧疗效果的客观指标,描记曲线,获得不同吸氧方式的量效曲线。结果获得不同吸氧方式流量等效曲线,以及各种状况的适用吸氧方式:(1)在小流量给氧时,三种给氧方式氧疗效果无明显差异;加大氧流量时,鼻塞效果更佳;(2)氧流量改变时,三种给氧方式氧疗效果响应速度依次为鼻塞吸氧〉面罩吸氧〉鼻管吸氧;(3)从舒适度、依从性上看,面罩吸氧〉鼻塞吸氧〉鼻管吸氧;(4)中等氧流量时,吸氧者感觉最为舒适,氧疗效果随流量增加而增强;(5)应用鼻塞吸氧,当流量为2~4L/min时,约等效于鼻管吸氧、面罩吸氧4~6L/min时的氧疗效果;(6)应用鼻塞吸氧,当流量为较大值时,可获得最佳氧疗效果;(7)当患者出现过度通气和低二氧化碳分压时,需应用面罩给氧,此时对比鼻塞给氧应加大氧流量,以达到满意疗效。结论经过监测获得了不同吸氧方式的量效曲线,为临床护理提供了有效参考。 Objective To investigate the effects for obtaining similar oxygen by different method, and to study the relationship between efficiency of oxygen therapy and the rate of oxygen flow. Method Normal parson and patients were divided into three groups by different oxygen method, such as oxygen mask, nasal oxygen tube and the oxygen snuffles respectively at different times (during shock, before and after operation ), and to adjust flux of oxygen and monitoring data of SPO2 and TcPO2 at diverse flux of oxygen by different oxygen method for dose-response curves. Result The suitable oxygen method was obtained by dose-response curves. (1) There was no significant difference in oxygen effect among three oxygen method at low flux of oxygen, when the flux of oxygen was increased, the best effect oxygen therapy was with the oxygen snuffles. (2) The speed of response for effect of oxygen therapy in turn was oxygen snuffles〉oxygen mask 〉nasal oxygen tube during oxygen flux changed. (3) For the degree of comfort, oxygen mask 〉oxygen snuffles 〉nasal oxygen tube. (4) The oxygen therapy is the best comfort when oxygen flux is middle. The efficiency of oxygen therapy is added following increased for flux of oxygen. (5) Nasal tube oxygen at 2-4 L/rain is equal to oxygen mask or the oxygen snuffles at 4-6 L/min. (6) The best effect is oxygen snuffles at high flux of oxygen. (7) As patient in hyperpnoea and low PCO2, mask and big flux of oxygen would.be given. Conclusion Dose-response curves of oxygen treatment was achieved by monitoring, which provided the base for clinical nursing.
