摘要
目的:探讨生殖免疫性抗体与不孕的关系,评价间接免疫荧光生物芯片技术在检测生殖免疫性抗体中的临床应用价值.方法:采用间接免疫荧光法(IIF)结合生物芯片技术,检测260对不孕夫妇和100对正常孕期夫妇血清中抗精子抗体(AsAb)等多项生殖免疫性抗体,同时采用ELISA法检测不孕组夫妇AsAb,并对结果进行比较分析.结果:不孕组各项抗体阳性率均高于对照组,比较有显著差异(P<0.01,P<0.05);AsAb阳性标本中IgM,IgG和IgA总阳性率分别为18.5%(15/81),79.0%(64/81)和2.5%(2/81);AsAb结合于精子头部、尾部、中部和整体的百分率依次为44.7%(36/81),37.0%(30/81),12.3%(10/81)和6.2%(5/81).IIF法检测AsAb的阳性率明显低于ELISA法,两者差异有统计学意义(15.6%vs20.4%,χA2sAb=11.7,P<0.01),但两者的符合率较好,为89.8%.结论:在不孕症的诊治中,生殖免疫性抗体的检测有着很重要的意义;间接免疫荧光生物芯片技术检测生殖免疫性抗体,既能定性、分型,又能定位,有较好的临床应用价值.
AIM:To study the relationship between antireproductive antibody and infertility, at the same time evaluate the clinical value of indirect immunofluorescent (IIF) and biochip technology in antireproductive antibody test. METHODS:Of 260 sterility patientshntisperm antibodies were tested in the serum with IIF and biochip technology. One hundred fertile people were choosed as control group. Meanwhile, sterility patients' antisperm antibodies were determined by ELISA, two results were compared and the results analyzed. RESULTS: The positive rate of antisperm antibody in the serum of infertile patients was much higher than that of normal control group ( P 〈 0.01, P 〈 0.05 ) ; The findings were 15 cases(18.5% ) IgM positive,64 cases(79. 0% ) IgG positive ,2 cases (2.5%) IgA positive respectively in antisperm antibodies(AsAb) positived group. Antisperm antibodies binding to the sperm head, midpiece, tail and all regions were 44.7% (36/81) ,37.0% (30/81), 12.3% ( 10/81 ) ,6.2% (5/81) respectively. The positive rate of IIF test was inferior to ELISA, and there were remarkable differences between the two methods (15.6% vs 20.4% x^2AsAb =11.7, P〈0.01). But the coincidence rate of the two methods was 89.8%. CONCLUSION:The location, type and property can be investigated simultaneously by the indirect immunofluorescent technique in anti-reproductive antibodyassay. So it was of higher clinical value in the diagnosis of infertility.
