原发性高血压合并阻塞性睡眠呼吸暂停综合征患者肾素-血管紧张素-醛固酮系统及胰岛素抵抗的改变 被引量:6

摘要 目的探讨原发性高血压合并阻塞性睡眠呼吸暂停综合征(OSAS)患者血浆肾素-血管紧张素-醛固酮活性及胰岛素抵抗(IR)的改变。方法将56例原发性高血压(EH)患者分为单纯EH组34例和EH合并OSAS组22例,测定其晨起卧位肾素活性(PR)、血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)及空腹血糖、空腹胰岛素,计算胰岛素抵抗指数(HOMA-IR)。结果单纯EH组和合并OSAS组ALD浓度差异有统计学意义[(67.2±41.5)ng/L与(125.6±43.2)ng/L,t=-2.51,P=0.02]。合并OSAS组HOMA-IR高于单纯EH组,差异有统计学意义[(1.4±0.4)与(O.3±0.3),t=-7.48,P=0.00]。结论合并OSAS患者存在着肾素-血管紧张素.醛固酮活性激活及胰岛素抵抗现象。
