摘要
目的观察应用腓肠神经营养血管逆行岛状皮瓣修复同侧小腿及足部软组织缺损的临床效果。方法应用腓肠神经营养血管逆行岛状皮瓣修复同侧下肢软组织缺损35例,皮瓣面积最大16 cm×11 cm,最小7cm×6 cm,其中将小隐静脉与受区静脉吻合14例,未吻合静脉21例。结果35例皮瓣完全成活,未吻合静脉的21例皮瓣中出现水疱6例,边缘坏死6例,部分坏死5例,经过对症处理后存活。结论应用腓肠神经营养血管逆行岛状皮瓣修复同侧小腿及足部软组织缺损简单实用,吻合静脉可改善皮瓣静脉回流以及提高皮瓣的成活率。
Objective To observe the clinical application of island fasciocutaneous flap pedicled with nutrient vessels of the aural cutaneous nerve to repair the soft tissue defects of the leg and foot. Methods 35 patients with the homonymy leg soft tissue defects were treated with the flap. The largest flap size was 16 cm × 11 cm,the smallest flap was 7 cm× 6 cm. The small saphenous vein were inosculated with the recipient site vein in 14 patients,while not in the other 21 cases. Results 35 flaps all survived, in 21 flaps with not inosculation vein:6patients appeared 6 lister, 6 patients edge necrosis. 5 patients part necrosis, all survived by symptomatic treatment. Conclusions The application of the island fasciecutaneous flap pedicled with nutrient vessels of the sural cutaneous nerve to repair the soft tissue defects of the leg and foot was simple and useful. The way to inosculation vein can improve vein recirculation and elevate the survival rate.
