摘要
目的探讨缺氧诱导因子-1α(HIF-1α)mRNA、缺氧诱导因子-2α(HIF-2α)mRNA在妊娠期肝内胆汁淤积症(ICP)患者胎盘组织上的表达及其与胎儿缺氧的关系。方法采用实时荧光定量法分别检测轻度ICP组、重度ICP组、正常妊娠组三组胎盘组织上的HIF-1αmRNA、HIF-2αmRNA的表达。结果35例胎盘组织均表达HIF-1α、HIF-2αmRNA,重度ICP组HIF-1αmRNA表达水平显著低于轻度组和正常组(P<0.05),且其表达与ICP分度之间存在等级相关(r=-0.409,P=0.015),轻度组与正常组差异无统计学意义(P>0.05)。HIF-2αmRNA表达水平三组间差异无统计学意义(P>0.05),其表达与ICP分度之间无等级相关性(r=-0.263,P=0.463)。结论ICP患者HIF-1αmRNA表达明显降低是机体适应慢性缺氧的表现,当在宫缩等急性缺氧应激下发生失代偿时可能是导致胎儿不良结局的原因。HIF-1αmRNA可作为判断病情程度的一个指标。
Objective To study the mRNA expression of hypoxia inducible factor-1α(HIF-1α) mRNA and hypoxia inducible factor-2α(HIF-2α) in the placenta of intrahepatic cholestasis of pregnancy and to find the relationship of their expressions and fetal anoxia. Methods HIF-1α mRNA and HIF-2α mRNA were measured by real-time fluorescent quantitation (RTQ -PCR) in the placenta tissues of the mild ICP pregnancy women, severe ICP pregnancy women and normal pregnancy women. Results HIF-1α, HIF-2α mRNA were expressed in all 35 placenta tissues. The expression in the placenta tissue of severe ICP pregnancy women was significant lower than that of the mild ICP pregnancy women and normal pregnancy women ( P 〈 0.05 ). There was significant correlation between the expression level and ICP scale division(r=-0.409 ,P = 0.015). There was no significant difference between the mild ICP pregnancy women and normal pregnancy women( P 〉 0.05) the expressions of HIF-2α mRNA in the placenta tissue was no significant difference among the three groups (P 〉 0.05), and there was no rank correlation between the expression level and ICP scale division(r=-0.263, P=0.463), Conclusion The lower expression of HIF-1α mRNA of ICP pregnancy women was a manifest of organism adaptation to chronic anoxia. The expression of HIF -1α mRNA is one of important indexes to judge the pathogenetic conditions of ICP pregnancy women.
