摘要
目的总结人工机械瓣膜置换术同期射频消融治疗风湿性心脏病合并心房颤动患者的术后护理要点。方法2007年1月-2008年12月本院收治的31例风湿性心瓣膜病合并心房颤动患者,给予机械瓣膜置换术同期射频消融术,术后给予针对性的护理。结果8例发生房室传导阻滞,4例发生房性早搏,5例发生心房颤动,30例使用临时起搏器;29例发生低钾血症;1例发生呼吸功能不全;11例出现少量心包积液;15例术后出现焦虑情绪,经处理后均康复出院。结论人工机械瓣膜置换术同期射频消融术可有效改善患者风湿性心脏病合并心房颤动;术后预见性防止患者心律失常及出血等并发症,密切观察患者病情变化,能有效促进患者康复。
Objective To summarize the perioperative nursing highhghts on maze-hne radiofrequency ablation for treating rheumatic hear disease （RHD） complicated with atrial fibrilation after artificial mechanical valve replacement. Methods 31 RHD patients combined with atrial fibrillation were treated with artificial mechanical valve replacement and maze-line radiofrequency ablation from January 2007 to December 2008. All of them was closely observed and carefully nursed and then followed-up after discharge. Results At the follow-up 6 months after discharged, 8 had auriculo-ventricular block, 4 had atrial premature beats, 5 had atrial fibrillation. 30 patients used the temporary pacemaker. 29 patients developed with hypopotassaemia and 1 patients developed with respiratory insufficiency. 11 patients had small amount pericardial effusion. 15 patients had anxiety. Conclusions Artificial mechanical valve replacement combined maze-line radiofrequency ablation can treat atrial fibrillation effectively for good. The prevention of postoperative bleeding and arrhythmia, close observation and careful nursing after operation can promote patients＇ recovery.
