三峡库区人群日本血吸虫病潜在流行因素分析 被引量:2

摘要 目的了解三峡库区当地人群日本血吸虫感染的潜在流行因素。方法选择三峡库区中段6个区县的当地居民为研究对象,用统一的调查表作个案调查,采集2~3mL静脉血检测人群血吸虫特异性抗体(IgG),进行单因素与多因素分析。结果本次调查3 003人,人群血吸虫抗体水平几何平均数(OD均值)为0.017 2。经单因素与多因素分析,非后靠移民的当地人群(R=0.260 6,P〈0.05)、饮用河沟水(R=0.215 6,P〈0.05)、以牛耕为主要耕作方式(R=0.237 3,P〈0.05)、家庭人均收入低(R=0.058 0,P〈0.05)、粪便未无害化处理(R=0.131 4,P〈0.05)、生产生活常接触水(R=0.079 5,P〈0.05)等因素与人群血吸虫病显著相关,为主要潜在流行因素。结论三峡库区人群存在血吸虫病发生和流行的潜在危险因素。建议进一步健全三峡库区血吸虫病监测体系,针对重点人群,开展健康教育和行为干预,预防和控制血吸虫病的发生和流行。 Objective To explore the potential epidemic factors of infection with schistosoma, japonicum of local inhabitants in the Thee Gorges reservoir areas (TGRA). Methods The local inhabitants were investigated in 6 counties of the middle part of TGRA. Each case was interviewed by recording data in uniform form. schistosomiasis IgG was examined by ELISA. The data was analyzed for single-factor and multi-element analysis. Results 3 003 people were investigated. The Geometric Mean of OD value (OD-value) of anti-schistosome antibody level was 0. 017 2. Univariate and multivariate analysis showed that the non-immigrate lo- cal inhabitants (R=0.2{50 6 ,P〈0.05) ,drinking river (R:0. 215 6 ,P〈0.05) ,farm style with cattle (R=0. 237 3 ,P〈0.05) ,low income (R = 0. 058 0, P〈0.05), non-decontamination of feces (R = 0.131 4, P〈0.05) and frequent contact-water while producing and living (R=0. 079 5,P〈0.05) were significant correlation with schistosomiasis. They were main potential epidemic factors of schistosomiasis transmission. Conclusion The potential hazard factors affecting schistosomiasis transmission exist in the TGRA. So the monitoring system of schistosomiasis should be improved and the health education should be carried out in key populations to prevent infection from S. japonicum in the TGRA.
