摘要
目的:利用血管回声跟踪技术(echo-tracking,ET)评价单纯老年人高血压患者颈总动脉弹性。方法:选择老年人单纯高血压患者60例作为病例组,健康同龄志愿者50例作为对照组;对受试者双侧颈总动脉行ET检测,获得颈总动脉僵硬度参数:硬度指数(β)、压力应变弹性系数(Eρ)、动脉顺应性(AC)和脉搏波传导速度(PWVβ),两组间对比,并不同级别高血压组之间作对比。结果:与对照组相比病例组β,Eρ和PWVβ明显升高(P<0.05),AC显著减低(P<0.05),高血压患者组内比较,3级者β、Eρ和PWVβ较1级者明显升高(P<0.05),1、2级者上述参数未见显著差异,AC参数高血压组内比较未见显著差异。结论:单纯老年人高血压使大动脉僵硬度显著增加,且僵硬度随着高血压级别升高有显著增加,用ET技术能准确显示此种变化,并能够量化此种病变。
Objective:To evaluate the carotid artery elasticity of the elderly with hypertension only using echo-tracking(ET) technique.Methods:Sixty patients with hypertension only of the elderly and fifty normal in the same age range were included and got the immediate blood pressure of every person.Performed the ET exam off the bifurcation 2 cm of the common carotid artery of every person and got the elasticity parameters:stiffness parameter(β),pressure-strain elasticity modulus(Eρ),arterial compliance(AC),and pulse wave velocity (PWVβ), and then compared all the parameters of the two groups and the patients group themselves. Results: Compared with the normal group, β,Eρ and PWVβ of the patients group were significantly higher( P 〈 0.05), AC was significantly lower ( P 〈 0.05) ,and in the patients groups themselves β, Eρ and PWVβ in the third stage were more higher than than of stage 1 ( P 〈 0.05), there was no significant difference between stage 1 and stage 2 of the four parameters, and there was no significant difference of AC in the three stages. Conclusion: Hypertension only to tile elderly could affect the elasticity of large artery seriously and deteriorate the elasticity and the higher stage of the hypertension of tile artery stiffness. The ET technique could detect the changes better than the formation change and quantify such lesions.
