磁共振波谱分析在帕金森病早期诊断中的应用研究 被引量:8

摘要 目的回顾性分析43例1H-MRS检查的帕金森病病例,探讨1H-MRS对帕金森病的诊断价值。方法以43位临床确诊的原发性帕金森病病人和30位年龄相仿的正常人为研究对象,对纳入的帕金森病患者进行UPDRS评分。对帕金森病患者及正常对照组豆状核进行单体素质子磁共振波谱扫描,获取磁共振波谱信号,计算NAA/Cr、Cho/Cr、NAA/Cho之比值。比较PD患者和正常人之间上述值的差异并进行统计学分析。结果早期帕金森病患者患肢对侧豆状核及中晚期帕金森病患者豆状核NAA/Cr、NAA/Cho较正常对照组明显降低(P<0.05),Cho/Cr比值变化不明显(P>0.05);但随着患者病情加重其豆状核的NAA/Cr、Cho/Cr、NAA/Cho比值变换不明显(P>0.05)。结论1H-MRS对帕金森病早期临床诊断和疾病病理生理研究具有良好前景。 Objective To explore the diagnostic value on proton magnetic resonance spectroscopy(1H-MRS) of idiopathic Parkinson s disease(PD). Methods Forty-three and thirty age matched healthy controls underwent the 1H-MRS scan in the area of putamen. A regular MRI was acquired to exclude the latent intracranial disease before the 1H-MRS scan. Then a Fourier convert was taken to reconstruct the two-dimensional graph including the relative concentration of the ratios of NAA/Cr,Cho/Cr and NAA/ Cho were calculated. Afterwards a statistical analysis was carried out to distinguish the difference of the ratios listed above between the PD patients and the controls. Results NAA/Cr,NAA/Cho ratios were reduced significantly (P〈0.05) in the striatum contralateral to the affected lmb of patients with early-stage PD and the both side of striatum of patients with middle and advanced PD,compared to those in age matched healthy control subjects.However,corresponding progressive changes in NAA/Cr,Cho/Cr and NAA/Cho ratios among the later stages of PD (H-Y Ⅱ,Ⅲ,Ⅳ) were not siguificant(P〉0.05). Con- clusion The single-voxel proton magnetic resonance spectroscopy is a valuable tool to detect the metabolic changes in the patient with Parkinson's disease.
