腹部开放伤合并海水浸泡致犬急性心力衰竭的实验研究

摘要 目的建立腹部开放伤合并海水浸泡犬急性心力衰竭模型,探讨海水浸泡对腹部开放伤犬急性心力衰竭的影响。方法成年杂种犬16只制备开放性腹部损伤动物模型,按随机数字表法分为海水浸泡组和对照组,每组8只。分别于致伤前、后,浸泡0.5、1、2h及打捞后1、2h监测2组犬的平均动脉压(mean artery pressure,MAP)、肺动脉嵌压(pulmonary artery wedge pressure,PAWP)、中心静脉压(central vein pressure,CVP)、心输出量(cardiacoutput,CO),检测血浆K+、Na+、Cl-、血气、脑钠肽(brain natriuretic peptide,BNP)、TNF-α含量,测定致伤前、打捞后2h心脏超声左心室收缩末容积(left ventricula rend-systolic volume,LVESV)、左心室舒张末容积(left ventricular end-diastolicvolume,LVEDV)、每博输出量(strokevolume,SV)、左心室射血分数(left ventricularej ectionfraction,LVEF)值。结果浸泡组犬在浸泡后MAP、CVP、CO均下降,至2h时,PAWP>18mmHg;LVESV、SV、LVEF均较对照组显著减低(P<0.05),血浆K+、Na+、Cl-、BNP、TNF-α含量均较对照组显著升高(P<0.05);浸泡组犬在实验期间酸中毒程度逐渐加重,PH、HCO3-进行性降低,与致伤前及对照组同时间点相比差异均有统计学意义(P<0.05)。结论海水浸泡是导致腹部开放伤犬急性心力衰竭的重要因素。 Objective To evaluate the effect of seawater immersion on acute heart failure in dogs with open abdominal wound and establish a relevant experimental model. Methods Sixteen health dogs were divided randomly into 2 groups,control group ( n = 8) only having abdominal wound,and seawater immersion group ( n =8) while abdominal wound was followed by seawater immersion. Mean artery pressure ( MAP) ,pulmonary artery wedge pressure ( PAWP) ,central vein pressure ( CVP) and cardiac output ( CO) were measured and blood samples were taken at different time intervals to determine K + ,Na + ,Cl ,blood gas,BNP and TNF-α levels. Doppler echocardiography was performed to measure the left ventricular end-systolic volume ( LVESV) , left ventricular end-diastolic volume ( LVEDV) ,stroke volume ( SV) ,and left ventricular ejection fraction ( LVEF) respectively. Results In seawater immersion group,MAP,CVP and CO were decreased after immersion. Compared with control group,LVESV,SV and LVEF were decreased and plasma levels of K + , Na + ,Cl ,brain natriuretic peptide ( BNP) ,TNF-α were increased significantly in seawater immersion group ( P〈0. 05) . In seawater immersion group,acidosis was aggravated gradually. pH value and HCO3were decreased significantly compared with control group ( P〈0. 05) . Conclusion Seawater immersion is a significant factor of acute heart failure in dogs with open abdominal wound.
