胰岛移植现状及展望

摘要 糖尿病(diabetes mellitus,DM)是一种严重威胁人类健康和生命的高发性的代谢性疾病。临床分为1型(T1DM)、2型(T2DM)、特殊型、妊娠糖尿病4种类型。目前常用的糖尿病治疗方法包括胰岛素注射、胰岛素泵、全胰腺移植和胰岛移植。虽然注射外源性胰岛素可以有效控制血糖,但无法改善血糖的生理性代谢和根本解决DM并发症的问题。而全胰腺移植和胰岛移植均可使受者获得接近生理状态的胰岛素分泌。与全胰腺移植相比,胰岛移植手术简单、安全、并发症少,可纠正糖代谢紊乱,尤为重要的是可以防止、减缓甚至逆转糖尿病性微血管病变,减少心、肾等脏器并发症的发生,降低病死率。近些年来,随着胰岛分离纯化技术提高、新型免疫抑制剂问世等,胰岛移植已从实验阶段进入了临床实践,取得了令人鼓舞的效果。
