摘要 目的:观察舒芬太尼用于腹腔镜胆囊切除术的血流动力学影响及麻醉恢复情况。方法:60例择期腹腔镜胆囊切除术患者,ASA分级I^II级,年龄36~72岁,均无高血压病史。随机分为舒芬太尼(SF)组和芬太尼(F)组。观察两组诱导前(T0)、气管插管即刻(T1)、气腹前(T2)、气腹后(T3)[压力达13mmHg(1mmHg=0.1333kPa)]5min的SBP、DBP、HR及术后麻醉恢复情况。结果:应用等效剂量SF和F,SF组SBP、DBP、HR均明显低于F组(P<0.05);两组自主呼吸恢复及时,F组初醒(呼之睁眼)时间早于SF组,而清醒及拔管时间晚于SF组(P<0.05)。结论:SF用于腹腔镜胆囊切除术能有效抑制术中应激反应,血流动力学稳定,麻醉恢复迅速平稳。 Objective: To observe the effect of sufentanil for laparoscopic cholecystectomy on hemodynamic and anesthesia recovery condition. Methods:Sixty ASA Ⅰ~Ⅱ patients (36-72years old) underwent selective laparoscopic cholecystectomy were divided into two groups randomly with sufentanil (group SF,n=30 ) and fentanyl(group F, n=30). SBP,DBP, HR were detected at different time points which including times before induction (T0), immediately intubation(T1 ), before pneumoperitoneum(T2 ), 5 minutes after pneumoperitoneum(T3 )(the pressure of CO2 Pneumoperitoneum was 13mmHg), meanwhile, anesthesia recovery condition were recorded at four different stages. Results: Sixty completed trial. The using of equivalent dose of sufentanil or fentanyl are suitable in laparoscopic cholecystectomy, SBP,DBP, HR in group SF was significantly less than those in the group F(P〈0. 05). The spontaneous breathing of both groups have no significant difference(P〉0. 05). The first time to wake up in group SF were longer than those of group F(P〈0. 05), but awake and extubation time was much shorter(P〈0. 05). Conclusion: Sufentanil could be used in laparoscopic cholecystectomy that has lighten stress reaction , maintained the stabilization of haemodynamics , with rapidly and timely anesthesia recovery.
