摘要
目的:探讨宫颈癌及正常宫颈T1灌注成像时间-信号曲线(TIC)的特点及灌注参数差异。方法:分别对符合选择标准的43例宫颈癌患者及23例临床排除宫颈疾病的患者行子宫常规MRI扫描及宫颈部MRI灌注成像检查,利用Functool获取每一个灌注层面感兴趣区灌注参数,并进一步利用Excel 2003软件取得每一个灌注层面感兴趣区校正TIC,比较宫颈癌与正常宫颈TIC差异及灌注参数差异。结果:43例宫颈癌、23例正常宫颈TIC走行大致相似,都有显著的上升支,峰值后曲线较平坦,为一个较长的平台期。但与正常宫颈相比,宫颈癌组TIC曲线上升支较陡峭,且较快到达峰值及平台期,但平台期维持时间相对较短;正常宫颈组与宫颈癌组增强高峰值(PH)无显著性差异(P=0.0687),宫颈癌组高强化区正性增强积分(PEI)、最大上升斜率(MSI)高于正常宫颈组(P分别为0.0010、0.0013),达峰时间(TTP)短于弱强化区(P分别为0.0250)。结论:TIC可从总体上更直观地描述宫颈癌与正常宫颈MRI血流灌注差异,而在此基础上得出的灌注参数可更准确地反映宫颈癌血流动力学特征。
Objective: To explore the characteristics of time-signal intensity curve (TIC) and the differences in perfusion parameter of uterine cervix cancer and normal uterine cervix. Methods: MRI routine scanning and perfusion imaging were performed in 43 patients with uterine cervix cancer and 23 patients without uterine cervix cancer. Data of MRI perfusion imaging in ROI were derivatized into perfusion parameter by Functool software package, and drew into revised TIC by Excel 2003, the differences of TIC and perfusion parameter in both groups were analysed. Results: In 43 patients with uterine cervix cancer and 23 patients without uterine cervix cancer, the shape of TIC and revised TIC of each group were similar, they showed an obvious steep up slope, a long plateau after peak value. Compared to normal uterine cervix group, the upslope of revised TIC of uterine cervix cancer groups were steeper; among MRI perfusion parameters, the median of the PH value had no difference between the normal cervix group and uterine cervix cancer group(P=0.0687), PEI and MSI in obvious enhancement areas of u- terine cervix cancer group were higher than those of normal cervix group(P=0.0010, 0.0013, respectively), but TIP were shorter(P=0.0250). Conclusion: TIC can directly describe the difference in blood perfusion between uterine cervix cancer and normal uterine cervix overall, and perfusion parameters can accurately reflect the hemodynamies characteristics of uterine cervix cancer.
