螺旋CT多期扫描测定肾小球滤过率不同扫描技术的比较 被引量:4

摘要 目的:比较不同扫描方法对螺旋CT多期扫描测定肾小球滤过率(glomerular filtration rate,GFR)的影响。方法:选择25例无急性肾功能障碍的成年患者,CT检查包括腹部平扫和双期增强扫描(动脉期、门静脉期),同时在双期增强扫描之间插入一组同层动态扫描。每例按不同计算方法将所得CT清除率数据分为二组:A组将动脉期和门静脉期间的一组同层动态扫描数据计算入主动脉时间-密度曲线(Time-density curve,TDC)积分;B组不把该组同层动态扫描数据计算入主动脉TDC积分。作为参照,以血清Cystatin C测定GFR作为实验室对照指标。结果:采用A组计算方法所得CT清除率与血清Cystatin C测定GFR的相关性更好,两者的相关性=0.80,直线回归方程y=39.0977+0.6083x。结论:在动脉期和门静脉期之间增加动态扫描层数,采集更多的扫描数据绘制主动脉TDC,可以提高螺旋CT多期扫描测定GFR的准确性。 Objective:To analyze the differences on measuring glomerular filtration rate by multiphasie Helical CT with different scan- ning technique. Methods:Twenty-five adult patients without acute renal disorder were included in this study. Each patient underwent CT for clinical indications. The CT protocol consisted of a plain examination followed by two eontxast material-enhanced examinations in the arterial and portovenous phase, 4 dynamic scans were inserted between the arterial and portovenous phase. GFR was calculated from CT data in 25 patients, the results were assigned 2 groups, i.e, group A, the aortic attenuation curve-also known as the input function-has to be determined for calculation of GFR. Four parts of the aortic attenuation curve were measured on CT images, including images ob- talned with 7 dynamic scans, and 4 dynamic scans obtained between the arterial and portovenous phase, and images obtained during the arterial phase and during the portovenous phase, group B, only except the 4 dynamic scans obtained between the arterial and portovenous phase. As a reference. Cystatin C was determined in 25 blood specimens. Results: Linear regression analysis showed that group A and Cystatin C had very well associativity, the correlation eoefficient of whole kidney was r = 0.80 and a regression line of y = 39.0977 + 0.6083x.C.ondusion:CT images were obtained with dynamic tectmiques between the arterial and portovenous phase can gain suflleient data for arterial input function, and GFR can be measured more accurately with multiphasic Helical CT.
