喉罩通气与气管插管在神经外科介入栓塞术麻醉中的比较 被引量:7

摘要 目的比较神经外科介入栓塞术应用喉罩通气麻醉和气管插管麻醉对围术期循环与通气功能的影响。方法择期行神经外科介入栓塞术成年患者36例,随机分为喉罩组(L组)和气管插管组(C组),每组18例。两组均采用静脉靶控输注(target-controlled infusion,TCI)咪唑安定,单次静脉注射芬太尼、丙泊酚、维库溴铵麻醉诱导,术中用TCI咪唑安定,吸入异氟醚,间断静脉注射维库溴铵维持麻醉。记录两组患者诱导前(T0)、插喉罩/气管导管时(T1)、手术开始时(T2)、手术进行30 min(T3)、拔喉罩/气管导管时(T4)的收缩压(SBP)、舒张压(DBP)、心率(HR)、脉搏血氧饱和度(SpO2)及熵指数的变化,记录插喉罩/气管导管后(TⅠ)、通气30 min(TⅡ)、通气60 min(TⅢ)、术毕(TⅣ)时呼气末二氧化碳分压(PETCO2)和平均气道压力(PAW)的变化,以及苏醒期表现。结果L组T1、T4的血压较T0升高,但差异无统计学意义(P>0.05),C组T1、T4的血压较T0升高明显(P<0.05),组间比较C组升幅明显(P<0.05)。L组T1、T4的HR增快幅度明显低于C组(P<0.05)。两组TⅠ、TⅡ、TⅢ、TⅣ时间点的PETCO2、PAW比较无明显差异(P>0.05)。L组拔管期过程平稳,呛咳、躁动、呕吐发生率明显低于C组(P<0.01)。结论在神经外科介入栓塞术中应用喉罩通气,患者插管期和苏醒期的血流动力学变化较气管插管麻醉平稳,能保证稳定通气,且苏醒期并发症少。
