摘要
目的探讨脂蛋白(a)[LP(a)]与糖尿病(DM)之间的关联。方法选择江苏省35-74岁多代谢异常和代谢综合征人群,以DM患者作为应变量,将LP(a)四等分作为自变量代入Logistic模型,分别进行单因素和多因素分析。结果糖尿病的患病率随着LP(a)水平的升高而增加;相关分析显示,LP(a)与甘油三酯(TG)的相关性最强,与低密度脂蛋白胆固醇(LDL-C)的相关性最弱,且除了LDL-C外与其他危险因素的相关系数差异均有统计学意义;在LP(a)与DM的单因素分析中,筛选出差异有统计学意义的指标:年龄、吸烟、血压、TG、载脂蛋白(APO)A/B;代入多因素模型后,LP(a)等分后的第二、三、四组与第一组比较的OR值分别为2.4,2.0和3.7,P〈0.001;采用多因素逐步调整Logistic模型分析发现,调整血脂指标后,OR值由6.6降为2.2,但差异仍有统计学意义。结论LP(a)是糖尿病的独立危险因子,LP(a)与糖尿病(DM)及其微血管病变及心血管并发症有联系。
Objective To explore the association of lipoprotein(a)with diabetes(DM).Methods The study was based on a population of Jiangsu province.The association of LP(a)with DM was analyzed in a cross-sectional study with 2 330 subjects aged 35-75 years.Results The prevalence of DM increased with LP(a) level.LP(a) correlated cloesly with triglycerides,loosely correlated with low-density lipoprotein cholesterol(LDL-C).No significant association was found between LP(a) and LDL-C.After adjusted for other factors,LP(a) was still significantly associated with DM.After adjusted for lipoproteins,the ORs for DM caused by LP(a)reduced from 6.6 to 2.2(P〈0.001).Conclusion LP(a) is an independent risk factor of DM.LP(a)may be one of the underlying pathogenic risk factors for DM and its complications.
