摘要
目的评价重型颅脑损伤(SHI)患者开颅术中低血压对预后的影响。方法回顾性分析263例SHI患者资料。行急诊开颅手术183例,对63例术中低血压患者进行复苏,并对预后进行评估。结果63例中,恢复良好的11例,中度残疾6例,重度残疾2例,植物生存1例,死亡43例,病死率68.3%,高于同期开颅术中无低血压表现者(病死率30.2%)。结论SHI术中出现低血压明显增加患者的病死率,对术中低血压进行积极防治可改善预后。
Objective To investigate the impact of hypotension during craniotomy on the prognosis of patients with severe craniocerebral injury (SHI) underwent operation. Methods A retrospective review was conducted over the craniotomy of in 183 cases with SHI, who received craniotomy. Resuscitation was carried out in 63 cases with hypotension during craniotomy and the prognosis was evaluated. Results Of 63 cases, 11 cases recovered well, 6 cases were with mild disability, 2 cases were with severe disability, 1 case lived in vegetative status, and 43 cases died with a mortality rate of 68.2%, wich was higher than that of 30.2% in SHI patients experienced no hypotension during the same period. Conclusion The mortality rate is significantly increased in SHI patients with hypotension during craniotomy and the prognosis of SHI cases can be improved effectively by preventing and treating low blood pressure during operation.
