摘要
目的通过对川崎病患儿血清基质金属蛋白酶-9(MMP-9)水平的测定,探讨MMP-9与川崎病及其冠状动脉损害(CAL)发生的关系。方法观察组为2005年1月-2006年12月在本院住院的23例川崎病患儿,急性期和恢复期各抽取患儿外周静脉血3mL,同时抽取12例其他发热性疾病患儿(发热对照组)及15例健康儿童(健康对照组)外周静脉血进行对照,采用ELISA法测定其血清MMP-9水平,采用二维超声心动图观察心脏冠状动脉病变,根据超声心动图检查结果分为CAL组(n=13)和无CAL组(n=10)。结果观察组患儿急性期血清MMP-9水平较发热对照组及健康对照组均显著增高(Pa<0.01),CAL组患儿血清MMP-9水平较无CAL组增高更为显著(P<0.01);恢复期无CAL组血清MMP-9水平与发热对照组及健康对照组比较均无显著性差异(Pa>0.05),CAL组血清MMP-9水平虽较急性期显著下降,但仍显著高于无CAL组患儿(P<0.01);发热对照组血清MMP-9水平亦较健康对照组显著增高(P<0.05)。结论MMP-9在川崎病急性期尤其伴CAL时表达显著增高;MMP-9可能参与了川崎病血管炎尤其是CAL的病理过程。
Objective To investigate the clinical significance of matrix metalloproteinase - 9 ( MMP - 9 ) in the pathogenesis of Kawasaki disease(KD) and its complications of coronary arterial lesions (CAL). Methods Twenty - three inpatients with KD were admitted in hospital from Jan. 2005 to Dec. 2006, whose venous blood samples were obtained during the acute and convalescent stage respectively, and CAL was detected with 2 - Dechocardiography. Twenty - seven venous blood samples ( 12 febrile children and 15 normal physical examination children) were collected as healthy controls. Serum levels of MMP - 9 were measured by enzyme - linked immunoadsorbent assay ( ELISA ). According to the results of echocardiography, children were divided into 2 groups:with CAL group (n = 13 ) and without CAL group (n = 10 ). Results During the acute stage, serum MMP - 9 levels were significantly higher in KD than those in controls ( Pa 〈 0.01 ). Furthermore, MMP - 9 levels were significantly higher in CAL group than those in without CAL group ( P 〈 0. 01 ). During the convalescent stage serum MMP- 9 levels decreased significantly in KD group, and in without CAL group the data were similar to those of controls, but in with CAL group, serum MMP - 9 levels were still significantly higher than those in without CAL group ( P 〈 0.01 ). The levels of MMP - 9 in febrile children were significantly higher than those in healthy controls (P 〈 0.05 ). Conclusions Over - expression of MMP - 9 in the acute phase of KD may be involved in the formation of CAL and pathogenesis of KD.
