摘要
目的:探讨血清IgE水平、外周血嗜酸细胞计数(EOS)及家族特应性体质调查在儿童咳嗽变异性哮喘(CVA)中的诊断意义。方法:分别测定咳嗽变异性哮喘患儿、哮喘发作患儿及同期我院体检的正常儿童各30例血清IgE水平,外周血嗜酸细胞计数,并调查受检者三代内特应性体质家族史。结果:①咳嗽变异性哮喘患儿发作期血清IgE水平为(525.39±252.29)IU/ml,显著高于缓解期(153.92±62.23)IU/ml,P<0.001,CVA两组水平均高于正常对照组(32.26±14.83)IU/ml,P<0.05。②咳嗽变异性哮喘发作期患儿外周血嗜酸细胞计数水平(1.11±0.48)×109/L显著高于缓解期(0.50±0.23)×109/L,P<0.001,CVA发作期和缓解期均高于正常对照组(0.13±0.07)×109/L,P<0.05。③CVA发作期血清IgE水平和外周血嗜酸细胞计数与哮喘发作期的(528.26±203.11)IU/ml和(1.16±0.43)×109/L之间比较无统计学意义(P>0.05)。④家族特应性体质调查咳嗽变异性哮喘组阳性26/30例(86.7%)显著高于正常对照组阳性7/30(23.3%)(P<0.01)。结论:联合检测外周血嗜酸细胞计数(EOS)、IgE水平及家族特应性体质调查对诊断咳嗽变异性哮喘有重要意义。
Objective: To explore the role of serum immunoglobulin E (IgE) ,eosinophils(EOS) and family-specific constitution in the diagnosis of children with cough variant asthma ( CVA ). Methods : IgE and EOS were detected in 30 patients with cough variant asthma,30 with asthma attack and another 30 of healthy children received medical examination in our hospital(controls). Each child received the survey on the history of family-specific constitution in their consecutive three generations in all groups. Results ①The concentration of IgE in children under cough variant asthma attack was significantly higher than that of the children in remission( 525.39 ±252.291) U/ml vs ( 153.92 ± 62.23 ) IU/ml, P 〈 0.001 ). Also,the IgE levels in both groups of CVA were higher as compared with the controls (32.26 ± 14.83 IU/ml,P 〈0.05 ). ②The levels of EOS in children with paroxysmal cough variant asthma were raised higher than those of patients in remission phase[ ( 1.11 ± 0.48 ) × 10^9/L vs. (0.50 ±0.23 )× 10^9/L, P 〈 0.001 ] , and the levels in both groups of CVA were higher in comparison with the controls [ (0.13 ± 0.07 ) × 10^9/ L,P 〈0. 05 ] . ③The concentration of IgE and EOS showed no differences between the patients with paroxysmal cough variant asthma and those under asthma attack (P 〉 0.05. ④The history svrvey suggested that 86.75 (26/30) of the cough variant asthma positive cases were correlated with the history of family-specific constitution, which was significantly higher than that of the controls (23.3% ,P 〈0.001 ). Conclusion: Detection of IgE and EOS and history-taking on family-specific constitution can be of great inlportance in diagnosis of children with cough variant asthma.
