摘要
目的:比较甲状腺癌术前肿瘤内钙化与血流的彩超征象的阳性率,分析两种征象对临床诊断的意义。方法:回顾本院近6年甲状腺癌患者70例,总结术前彩超观察到肿瘤内钙化与血流信号,比较两种征象的出现频率有无联系及差异。结果:70例患者共切除术前彩超探测到的肿瘤87个,病理报告示:乳头状癌68个,滤泡状癌7个,髓样癌12个。术前超声报告肿瘤内有血流信号的29个(33.3%),有钙化的19个(21.8%),两种征象都有的24个(27.6%),两者都无的为15个(17.2%)。两种征象无相关性(P>0.05),且在全部肿瘤中出现的比率无差别(P>0.05),但45岁以上患者癌肿中,超声示有血流的肿瘤(39%)比率高于超声显示钙化的肿瘤(12.2%)(P<0.05)。结论:甲状腺肿瘤术前彩超检查,血流与钙化这两种征象对甲状腺癌有相同的诊断价值,可指导甲状腺肿瘤术中的切除范围。45岁以上的患者,肿瘤内血流信号更具有意义。
Objective：To analyze the preoperative urltrasonic image of thyroid carcinoma,and compare rate of calcification with blood supply in tumor,in order to determine their relation in clinical diagnosis.Methods：Seventy cases of thyroid carcinomain in our hospital during 2003-2009,were examined by colored ultrasonography preoperatively.Resalets：87 tumors were detected and excised in the seventy cases.Their patholgyoreport was：68 papillary carcinomas,7 follicular carcinomas,12 medullary carcinomas,respectively.Preoperative ultrasonography presented that blood flow was detected in 29 tumors（33.3%）and calcification was detected in 19 tumors（21.8%）,both in 24（27.6%）,neither in 15（17.2%）.In all tumors,the occurency frequency had no significant difference between blood flow and calcification in ultrasonography（P〉0.05）.and there was no correlation between the two ultrasonic images（P〉0.05）.However,in patients of over fourty-five-years old.the rate of tumors with blood flow（39%） is more than those with calcification（12.2%）（P〈0.05）.Conclusion： Ultrasonic image of blood supply is as important as ultrasonic image of calcification in the preoperatively diagnosis of thyroid carcinoma,and can also conduce to determining the extent of thyroid neoplasm dissection.in patients of over fourty-five-years old,ultrasonic image of tumors with blood flow is more significant.
