微创经皮肾镜取石术后不留置肾造瘘管法的探讨 被引量:3

摘要 目的探讨微创经皮肾镜取石术(MPCNL)中不留置肾造瘘管的可行性。方法对112例MPCNL术后患者的临床资料进行回顾性分析,并随机分成两组:A组(n=56)术后不留置肾造瘘管,B组(n=56)术后留置肾造瘘管,比较2组患者需用镇痛药物例数、术后住院时间、出血量、拔除导尿管时间、漏尿率。结果112例患者均Ⅰ期手术成功;A组需使用镇痛药者2例,B组需使用镇痛药者12例(P<0.05);术后住院时间A组5~7d,B组8~10d(P<0.05);A组术后出血量40~70mL,Hb下降4~18g/L,B组术后出血量60~130mL,Hb下降9~22g/L(P<0.05)。A组、B组拔除导尿管时间分别为4d、2d(P<0.05)。A组无1例漏尿,B组有2例漏尿。结论在MPCNL术中选择性地不留置肾造瘘管,术后痛苦少、恢复快、患者经济负担减轻、恢复时间缩短、术后出血量减少,是安全可行的治疗方法。
