脑淋巴阻滞后SAH大鼠脑组织含水率和血管内皮细胞损害及银杏内酯的影响

摘要 目的探讨脑淋巴引流阻滞(CLB)及银杏内酯对大鼠蛛网膜下腔出血(SAH)脑组织含水率与血管内皮超微结构的影响。方法选雄性成年Wistar大鼠,采用枕大池内新鲜自体动脉血二次注入法建立大鼠SAH模型,用颈淋巴管结扎和颈淋巴结摘除法制作大鼠CLB模型。将动物分为:正常对照组,SAH组,SAH+CLB组,SAH+CLB+生理盐水组(NS),SAH+CLB+20mg/kg银杏内酯组(Gl20组)和SAH+CLB+80mg/kg银杏内酯组(Gl80组)6组。干-湿重比较法测定脑组织含水率,透射电镜观察毛细血管内皮超微结构。结果脑组织含水率SAH组较正常对照组有所增加,SAH+CLB组较SAH组增加明显(P<0.01),SAH+CLB+Gl组与SAH+CLB组比较均明显降低(P<0.01),SAH+CLB+Gl80组较SAH+CLB+Gl20组有所降低(P<0.05),对照组以外各组神经元及毛细血管内皮超微结构均有不同程度的损伤,SAH+CLB+Gl80组损伤明显减轻。结论CLB可增加SAH脑水肿,并能加重脑内血管内皮的损伤。银杏内酯可降低脑淋巴阻滞后SAH大鼠的脑组织含水率,对血管内皮细胞具有保护作用。 Objective To determine the effect of cerebral lymphatic blockade (CLB) and ginkgolides on brain water content and ultrastructure of vascular endothelia in rats with subarachnoid hemorrhage (SAH). Methods Freshly autologous arterial blood was injected twice into cistern magna of 90 male mature Wister rats for inducing SAH, and CLB model in rats was established by occlusion of cervical lymphatics and removal of cervical lymphatic nodes. All these rats were divided into normal group, SAH group, SAH + CLB group, SAH + CLB + normal saline (NS) group, SAH + CLB + 20mg/kg ginkgolides group ( G1 20) and SAH + CLB + 80mg/kg ginkgolides group ( G1 80). Brain water content was detected by comparing dry with wet weight method. Electron microscopy was used for observing ultrastructure of vascular endothelia. Results SAH can increase brain water content. Increment of brain water content in SAH + CLB group was more severe than that in other groups ( P 〈 0.01 ). Brain water content in G1 groups was remarkably dropped to contrast one in SAH + CLB group( P 〈 0.01 ). The ultrastructure of vascular endothelia of rats except in negative control group was destructed in various extent. The impairment of vascular endothelia in G180 group was obviously decreased. Conclusion Brain lymphatic drainage pathway may play an important role in pathogenesis of cerebral edema and impairment of ultrastructure of vascular endothelia after SAH. Ginkgolides may relieve cerebral edema and impairment of vascular endothelia after SAH and CLB.
