摘要 目的研究腋臭患者大汗腺的组织学结构及分析腋臭的病因,为小切口真皮深层摘除大汗腺治疗腋臭的手术方法提供病理依据及探讨其效果。方法腋臭患者和腋部皮肤20例,正常人群腋部皮肤8例,肉眼观察皮肤的特征,并制成光镜观察的标本。在显微镜下观察两组标本的大汗腺分布层次、量、结构和功能。对121位腋臭患者采用小切口真皮深层摘除大汗腺的手术方法。结果大汗腺位于真皮深层和皮下脂肪浅层,与对照组比较,腋臭组大汗腺的量显著增多,腺上皮呈柱状或立方细胞,可见断头分泌,腺腔明显扩大,分泌物积聚。对121例患者经6-18个月随访,总有效率为100%,所有患者腋区皮肤无坏死,切口隐蔽,瘢痕不明显。结论大汗腺组织学的改变是产生腋臭的主要病因;小切口真皮深层摘除大汗腺是治疗腋臭的一种损伤小,疗效佳的方法。 Objective This study was designed to investigate the etiology of the axillary osmidrosis by studying the histological structure of the apocrine glands and to evaluate the effect of excision of apocrine glands under the deep dennis via a small transverse incision. Methods Histologically, biopsy samples from 20 patients with osmidrosis and 8 healthy volunteers were observed both macrographically and microscopically to compare the character of the skin and the distribution, density, structure and function of the apocrine glands. Clinically, 121 patients with axillary bromidrosis underwent excision of apocrine glands under the deep dennis and all these patients were followed-up for long-term results. Results The apocrine glands of the axillary were located at the deep layer of the dennis and superficial layer of the subcutaneous tissue. Compared with normal people, the density of the apocrine of the osmidrosis patients was significantly higher. Glandular epithelia were columnar or cubical. Decapitated gland with enlarged lumen and accumulated secretion were discovered. For those 121 patients underwent excision of apocrine gland under the deep dermis, the total effective rate was 100% after a follow-up period ranging from 6 to 18 months with well-concealed inconspicuous scars and no axillary skin necrosis. Conclusion Apocrine gland played the most important role in the etiology of osmidrosis and excision of apocrine glands under the deep dermis via a small transverse incision might be the surgical treatment of choice for axillary osmidrosis with its high effectiveness and minimal invasiveness.
