高血压病与糖尿病的5种尿蛋白联合检测的临床应用

摘要 探讨尿α1-MG、β2-MG、THP、Alb与IgG联合检测的临床应用。用放射免疫分析(RIA)检测50名正常人和144例高血压病与113例糖尿病患者的上述5种尿蛋白排泄水平,并与内生肌酐清除率(CCR)进行比较。CCR>100mL/mim时,α1-MG的排泄已显著增高(P<0.01);CCR<100mL/mm时,其余4种蛋白排泄显著改变(P<0.01);这5种尿蛋白与CCR均呈相关关系(P<0.01)。这5种尿蛋白均系诊断早期肾损害的敏感指标,尤以α1-MG最敏感。这5种尿蛋白联合检测可提高早期肾损害的检出率。 The objectives were to investigate the clinical value of urinary excretive levels of alpha -1- microglobulin (α1 - MG), beta - 2-microglobulin (β2-MG), Tamm - Horsfall protein (THP), albumin (Alb) and immunoglobulin G (IgG) in 144 patients with essential hypertension (EH) and 113 patients with non - insulin - dependent diabetes mellitus (NIDDM), and to  compare the serial microprotein levels and the correlation between them and the endogenous creatinine clearance rates (CCR). Radioimmunoassay (RIA) was used. The results showed that there was a good correlation between the urinary microprotein and CCR. When CCR > 100mL/min, the urinary α1- MG levels have been elevated obviously (P < 0. 01 ). The other four urinary microproteins have changed only when the CCR< 100mL/min. The five urinary microproteins were sensitive indexes for detection of early renal damage, and α1- MG was the most sensitive among them.
