摘要
目的探讨c-KIT、血小板衍生生长因子受体-α(PDGFR-α)和PDGFR-β蛋白表达在肺神经内分泌癌发生中的作用及与预后的相关性。方法采用免疫组化方法检测80例肺神经内分泌癌组织中c-KIT、PDGFR-α及PDGFR-β蛋白表达情况,分析三项指标间及与肿瘤临床病理参数、患者预后(Kaplan-Meier生存分析)的关系。结果c-KIT、PDGFR-α和PDGFR-β蛋白阳性表达率分别为45.00%、58.75%和66.25%,两两比较P均>0.05;三项指标与患者年龄、性别及肿瘤直径、分期均无明显相关性(P>0.05);除小细胞癌与类癌外,PDGFR-α蛋白在四种类型肺癌中的表达阳性率均有显著差异(P均<0.05);PDGFR-β蛋白在不典型类癌和大细胞癌中的表达有显著差异(P<0.05);PDGFR-α蛋白在大细胞癌、c-KIT蛋白在类癌、PDGFR-β蛋白在不典型类癌中表达阳性与阴性者中位生存期有显著差异。结论c-KIT、PDGFR-α及PDGFR-β蛋白表达在肺神经内分泌癌发生中具有一定作用,并与预后有关。
Objective To investigate the role of c-KIT,platelet-derived growth factor receptor(PDGFR)-α and PDG- FR-β protein expression in the occurrence and development of neureendocrine lung cancer,and analyze its correlation with the prognosis. Methods The expression of c-KIT, PDGFR-α and PDGFR-β protein in 80 cases of neureendocrine lung cancer were detected by immunohistechemistfieal method , then the correlation among the three indicators, and which with tumor clinicopathologic parameters,prognosis (Kaplan-Meier survival analysis)were analyzed. Results The expression of c-KIT, PDGFR-α and PDGFR-β protein were 45.00% ,58.75% and 66.25% (P 〉 0.05 ). There were no significant correlation between the three indicators and patient's age, sex,tumor size or stage(P 〉 0.05). The expression of PDGFR-α protein showed significant differences( P 〈 0.05) among four types of lung cancer except small cell carcinoma and carcinoid. There were significant differences between the expression of PDGFR-β protein in atypical careinoid and that in large cell carcinoma ( P 〈 0.05). To the expression of PDGFR-α protein in large cell carcinoma patients, c-KIT protein in carcinoid patients, PDGFR-β protein in atypical careinoid patients, there were significant differences of middle -survival -time between the positive and negative individuals(P 〈 0.05). Conclusion The expression of c-KIT, PDGFR-α and PDGFR-β protein play certain roles in the occurrence of neuroendcerine lung cancer, and are correlated to the prognosis.
