摘要 目的探讨极低出生体重儿(VLBWI)经腋静脉留置经外周置入中心静脉导管(PICC)的可行性。方法:2001年1月至2009年1月住院的97例VLBWI,其中65例经贵要静脉、32例经腋静脉留置PICC。置管时观察患儿心率、呼吸情况。观察并记录PICC导管穿刺的难易程度、是否一次成功,记录静脉炎、导管堵塞和异位等主要并发症的发生情况及导管留置时间。对两种穿刺途径的PICC置管效果进行对照和统计分析。结果:留置过程患儿生命体征平稳,两组穿刺时的问题、并发症的发生率以及留置时间差异无显著性。结论:VLBWI经腋静脉留置PICC导管的难易程度和并发症的发生率与经贵要静脉比较差异无显著性。在贵要静脉显露不清晰时,选择腋静脉留置PICC是可行的。 Objective Probing the feasibility of peripherally inserted central venous catheters ( PICC ) through axillary vein in very low birth weight infant ( VLBWI ). Methods PICC was placed through basilic vein in 32 VLBWI ( BV group ) and through axillary vein in 65 VLBWI ( AV group ). Their heart rate respiration and oxygen saturation were observed in both groups during inserting catheters. Patients were followed throughout their entire catheter course and the total duration of PICC and PICC-related complications including dislodgment, infection, occlusion, leakage were documented. Catheter outcome of two groups were compared. Results The vital signs of all patients were normal during placing catheters. The difference between the two groups was not statistically significant in the incidence of complications and total durations of PICC. Conclusions It is feasible to place PICC in VLBWI. When the basilica vein is not clear or puncture through it fails the axillary vein can be selected.
