摘要
目的探讨肱骨近端锁定钢板(LPHP)治疗肱骨近端粉碎性骨折的疗效。方法2006年2月～2008年2月,采用肱骨近端锁定钢板对23例肱骨近端粉碎性骨折患者进行治疗,其中男16例,女7例;年龄16～73岁,平均46.6岁;左侧8例,右侧15例。17例为摔伤,6例为车祸伤。根据Neer分型,2部分骨折(Ⅱ型)12例,3部分骨折(Ⅲ型)6例,四部骨折3例(Ⅳ型)。结果所有患者均获随访,时间6～24个月,平均18个月。平均骨折愈合时间平均6.3个月。1例出现肱骨头坏死,1例出现螺钉切割脱出,无1例出现腋神经损伤。所有患者均能完成日常生活动作。术后功能评价采用Constant评分表评价,优良率为86.9(20/23)。结论肱骨近端锁定钢板治疗肱骨近端粉碎性骨折效果好,可以达到骨折功能愈合及早期功能锻炼,特别对老年骨质疏松者尤为适用。
Objctive To explore the clinical results of proximal humeral fractures treated by proximal humeral locking plates. Methods From February 2006 to February 2008,23 patients with proximal humerus fracture were treated with a LPHP,16 male and 7 female,average age was 46.6 years ranging from 16 to 73.Fracture location： left 8, right 15;Fracture cause：17 fall injury, 6 traffic accident . According to Neer classification,12 cases were two-part fractures, 6 throe-part fractures,3 four-part fractures.Results All of the patients were followed up from 6 months to 24 months with average of 18 months.All cases average time of bony union was 6.3 months. No auxiliary nerve injury and one humerual head osteoneeosis,Aceording to constant scoring,the excellent and good result rate was 86.9（20/23）.Conclusion Treatment of Proximal humeral Comminuted fracture with locking plate shows good resuhs.h can achieve bony functional healing, early rehabilitation training and good shoulder function.especially fot the elderly patients with osteoporosis.
