摘要
目的观察白癜风自体表皮移植联合NB-UVB的疗效及用共聚焦激光扫描显微镜观察移植联合光疗前后的皮肤黑素环变化。方法自体表皮移植术治疗白癜风患者,每次移植前后用照片记录皮肤颜色变化,用共聚焦激光扫描显微镜观察移植白斑区域照射NB-UVB前后皮肤黑色素含量及黑素细胞形态的变化。结果20例联合NB-UVB治疗后的患者1次移植总显效率76.74%,有效率89.92%,2次移植显效率83.58%,有效率95.52%,2次移植显效率及有效率较1次移植稍高,两者差异无显著性(P>0.05),23例未联合NB-UVB的患者显效率73.34%,有效率82.98%,与照光组1次移植相比,差异无显著性(P>0.05),与照光组2次移植相比,差异有显著性(P<0.05)。20例照光患者中15例照光3次后移植处白斑边缘有缩小,色素加深,共聚焦激光扫描显微镜观察移植皮肤的黑色素环逐渐形成,接近正常。结论NB-UVB光疗联合自体表皮移植可以促进移植后皮肤黑色素生成,移植后的肤色较均匀。应用共聚焦激光扫描显微镜可以直观的观察到皮肤黑色素的形态变化,黑素环亮度加深与临床肤色加深较一致。
Objective To evaluate the usefulness of combination treatment of autologous epidermal graft and NB-UVB therapy and to observe the melanin of skin using CLSM. Methods Using the digital camera to record the pictures of the white maculae before and after therapy. Confocal laser scanning microscopy was used to observe the melanin of the skin before and after NB-UVB treatment. Results The effect of patients treated with NB-UVB was 89.92%, which was less than the effect of second graft(95.52% ). The patients treated only with graft was lower than effect of the combination group. The confocal laser scanning microscopy can show the melanin as the same as normal. Conclusions NB-UVB combination with autologous graft can promote the generation of the melanin. The confocal laser scanning microscopy might directly observe the melanin of the melanocyte, which was the same as the clinical repigmentation.
