三维适形放疗治疗胆管癌的预后因素分析 被引量:2

摘要 目的分析三维适形放疗(3DCRT)治疗不能手术的胆管癌的预后因素。方法47例胆管癌采用3DCRT治疗,总剂量30-70 Gy。结果照射剂量〈50 Gy组、50-59 Gy组、≥60 Gy组2年总生存率分别为0、48%和19%。〈50 Gy组2年总生存率低于50-59 Gy组和≥60 Gy组,差异有统计学意义(P〈0.01);而50-59 Gy组和≥60 Gy组的2年总生存率比较差异无统计学意义(P〉0.05)。3DCRT治疗前血清白蛋白正常组和血清白蛋白降低组2年总生存率分别为41%和0,差异有统计学意义(P〈0.01)。结论生存率的高低与照射剂量的大小及3DCRT治疗前血清白蛋白含量高低有关,3DCRT治疗前血清白蛋白含量是影响生存率的独立因素。 Objective To analyze the prognostic factors for inoperable cholangiocarcinoma treated with three dimensional conformal radiotherapy(3DCRT).Methods 47 patients were treated with 3DCRT.Doses of 3DCRT ranged from 30-70 Gy.Results 2-year overall survival rates of the patients given〈50 Gy,50-59 Gy,and ≥60 Gy were 0,48% and 19%,respectively.2-year overall survival rates of the patients given 〈50 Gy was lower than that of the patients given with 50-59 Gy,and ≥60 Gy(P〈0.01).The difference in 2-year overall survival rates between the patients given with 50-59 Gy and ≥60 Gy was not statistically significant(P〉0.05).2-year overall survival rates of the patients with normal and lower albumin values were 41% and 0,respectively.The difference in 2-year overall survival rates between the patients with normal and lower albumin values was statistically significant(P〈0.01).The albumin value before 3DCRT was the independent prognostic factor(P〈0.05).Conclusion The survival rates are related with the radiation dose and the albumin value before 3DCRT.The albumin value before 3DCRT is an independent prognostic factor for the survival rates.
