摘要
目的:研究先兆流产(胎漏、胎动不安)中医证型分布情况,揭示其证候分布规律。方法:基于证候形成规律,根据中医辨证标准,结合临床实际拟定肾虚型、脾肾两虚型、气血虚弱型、血热型、跌仆伤胎型5个证型。通过598例先兆流产患者证型分布情况的统计分析,探讨本病证型分布规律。结果:①先兆流产(胎漏、胎动不安)各证型分布有差异(P<0.05),肾虚为主者占50.6%。②各证型间妊娠继续率有统计学差异(P<0.05),血热型妊娠继续率最高(94.0%)。结论:①先兆流产(胎漏、胎动不安)辨证分型以肾虚型、脾肾两虚型、气血虚弱型、血热型、跌仆伤胎型5个证型为主,其中又以具肾虚症状者为多,提示"肾主生殖"在胎漏、胎动不安中的本质地位。②血热型在证型分布及妊娠继续率上均占优势,故血热伤胎亦为胎漏、胎动不安的重要因素之一。
Objective:To observe the distribution of different TCM syndrome types of threatened abortion(excessive fetal movement,vaginal bleeding during pregnancy) acording to chinese traditional medic pattern identification.Methods:598 cases of threatened abortion were divided into five groups according to different syndrome types of TCM:the type of deficiency of kidney,the type of deficiency of kidney and spleen,the type of deficiency of qi and blood,the type of blood heat,the type of blood stasis due to injure.Their pregnancies continued to the second trimester of pregnancy. Thourgh analyzing,this study explored the distribution of different TCM syndrome types of threatened abortion.Results:①The threatened abortion (excessive fetal movement,vaginal bleeding during pregnancy) was not equally divided into the five TCM syndrome types(P0.05),the kidney deficiency (the type of deficiency of kidney,the type of deficiency of kidney and spleen)was the main syndrome(50.6%).②The five TCM syndrome types shared different rates of threatened-continuing pregnancies(P0.05),the type of blood heat was the highest (94.0%).③There was no difference about the age and abortion frequency in five TCM syndrome types.Conclusion:①The deficiency of kidney was the main syndrome of threatened abortion(excessive fetal movement,vaginal bleeding during pregnancy). There was no difference about the age and abortion frequency in different TCM syndrome types. ②Blood heat may explain some reason for the threatened abortion.
