青年乳腺癌中ER、PR、HER-2及VEGF表达及其临床病理学意义 被引量:11

摘要 目的探讨青年患者乳腺癌雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER-2)及血管内皮生长因子(VEGF)表达情况以及与临床特点和病理学特征的关系。方法收集63例临床病理资料完整的青年乳腺癌病例(年龄在35岁以下),采用Envision试剂盒行免疫组化两步法检测癌组织中ER、PR、HER-2及VEGF表达情况,并与患者TNM分期、腋淋巴结转移状况进行比较分析,选取同期35岁以上中老年妇女乳腺癌患者390例进行对照。结果青年组乳腺癌ER、PR、HER-2及VEGF阳性表达率分别为41.27%、39.68%、68.25%、61.90%,中老年组分别为63.08%、52.31%、42.56%、43.33%,青年组ER阳性率低于中老年组(2=10.751,P<0.01),HER-2及VEGF阳性率高于中老年组(2=14.404,P<0.01;2=7.533,P<0.01);青年组ER、PR与TNM病理分期、腋淋巴结转移无相关性(P>0.05),HER-2、VEGF与TNM分期、腋淋巴结转移呈正相关(r=0.421,P<0.01;r=0.368,P<0.01),随着TNM分期增加及淋巴结转移数目的增多,两者阳性表达率明显升高。结论青年乳腺癌较中老年乳腺癌侵袭性强、预后差;乳腺癌组织中HER-2、VEGF表达与乳腺癌的TNM分期、腋淋巴结转移情况存在一定的相关性,可作为乳腺癌风险评估的重要指标。
